摘　　要：目的探讨创伤性关节炎患者X线分期与患者关节液中TNF-α、IL-6、IL-8水平和关节功能之间的潜在关系。方法选择2015年9月至2016年9月在(第三军医大学第一附属医院治疗的90例膝关节外伤的创伤性关节炎患者作为研究对象,以90例健康体检者为对照组,检测两组患者关节液中炎症因子水平,探讨关节液中炎症因子水平(TNF-α、IL-6、IL-8)与患者X线分级和关节功能的关系。结果创伤性关节炎患者关节液中TNF-α、IL-6、IL-8水平高于对照组(P<0.05),X线提示创伤性骨关节患者Ⅰ级37例、Ⅱ级23例、Ⅲ级21例、Ⅳ级9例,随着骨损害水平加重,TNF-α、IL-6、IL-8水平越高且组间差异有统计学意义(P<0.05),患者HSS评分和KSS评分与患者关节X线分级、关节液中TNF-α、IL-6、IL-8水平呈负相关(P<0.05)。结论创伤性关节炎患者X线分期与患者关节液中炎症因子表达及关节功能密切相关。Objective To study the relationship between the expression of inflammatory factors and joint function in patients with traumatic arthritis and the expression of inflammatory factors in joint fluid.Methods 90 cases of knee joint trauma in our hospital for the treatment of traumatic arthritis patients from September 2015 to September 2016 as the object of study,90 healthy volunteers as control group,two groups were detected in the synovial fluid of patients with inflammatory factors,to explore the level of inflammatory factors in synovial fluid(TNF-α,IL-6,IL-8)in patients with X-ray classification and joint function.Results TNF-α,IL-6and IL-8levels in the synovial fluid of patients with traumatic arthritis were significantly higher than that in healthy adults(P<0.05),radiographic evidence of traumatic osteoarthritis in patients with gradeⅠ37cases,Ⅱ23cases,Ⅲ grade 21 cases,9cases of gradeⅣ,with bone damage aggravated statistically significant TNF-α,IL-6,IL-8level is high and the differences between groups,patients with HSS score and KSS score in patients with joint X-ray classification,TNF-αand IL-6in synovial fluid and IL-8level is negatively related to(P<0.05).Conclusion The expression of inflammatory factors and joint function in patients with traumatic arthritis were closely related to the expression of inflammatory factors.
