摘 要:目的探讨气管切开插管留置大鼠不同时间段β-防御素-2(rBD2)及相关免疫指标的变化及其规律。方法将无特定病原体(SPF)级健康雄性Sprague—Dawley(SD)大鼠分成正常对照组(A)、气管切开插管留置模型组(B)各24只。制备大鼠气管切开插管留置模型。在造模成功后第24、72、168小时3个时段处死大鼠,观察体重及肺组织形态学改变。采集肺部及外周咀标本,检测rBD2 mRNA的表达、白细胞介素-2(IL-2)及T细胞亚群(CD3^+、CD4^+、CD8^+)的含量。结果(1)B组大鼠肺组织rBD2 mRNA在第24小时表达为2.230±0.521,与A组(0.767±0.269)比较明显升高,而后随着时间的延长其表达(0.229±0.098)逐渐下降,并低于A组(0.909±0.342)水平,至第168小时达到本时间段内最低值(0.107±0.054)。(2)B组外周血CD3^+含量在第24小时为(10.190±0.876)%,与A组含量(14.390±1.393)%比较明显下降,在第72小时含量(14.040±1.331)%,又恢复至A组大鼠(14.090±1.440)%水平,此后再没有改变。(3)血清IL-2及外周血CD4^+、CD8^+的含量,两组比较在3个时间段没有差异,同一组内在3个不同时间段之间的比较也没有差异。(4)B组大鼠体重随着时间的延长逐渐下降,并在第168小时下降最明显[(198.500±8.440)g]。(5)肺组织出现不同程度病变。结论气管切开插管留置大鼠免疫功能变化以肺部局部功能下降为主,这种变化可能与气管切开后肺部损伤和营养不良有关。Objective To investigate the variation in different time period and its regulation of the respiratory system beta- defensin -2 (rBD2) and related immune indexs in rats model with tracheotomy intubation by one -time epidural catheter. Methods Sprague -Dawley (SD) rats were randomly divided into 2 groups with 24 in each group: sham - operated group, model group. Observeing the changes of the weight and lung tissues in rats After modeling. And then, peripheral blood and lung tissues were collected with in 24h, 72 h and 168h respectively, lung tissues rBD2 mRNA expression were measured by reverse transcription - polymerase chain reaction ( RT - PCR), senun Interleukin - 2 ( IL - 2) content were measured by ELISA, and the peripheral blood T lymphocyte subsets were measured by Flow cytometry respectively. Results I,ung tissues rBD2 mRNA expression (2. 230 ± 0. 521 ) in 24 h increased significantly after modeling, and then began to decline, in the 72 h (0. 229 ± 0. 098) was lower than that in group A (0. 909 ±0. 342), in the 168 h to A minimum (0. 107 ±0. 054). Peripheral blood CD3^+ content (10. 190 ±0. 876)% in 24 h decreased significantly after modeling, back to group A level (14. 040 ± 1. 331 )% in the 72 h, since there is no change. But Serum IL -2 content and peripheral blood CD4^+ , CD8^+ content in group B compared with group A did not differ between three periods, and there was no difference between three differcnt time periods within the same grnup. Group B rats weight ( 198.5 ±8.44) g gradually decline, the lungs appcar different degree of pathological changes. Conclusion The changes of immune function in rats model with tracheotomy intubation by one - time epidural catheter were mainly localized in lung function, which may be related to lung injury and malnutrition after gas cutting.
