乙酰肝素酶在早孕绒毛组织与早期自然流产绒毛组织中的表达变化

摘　　要：目的:观察乙酰肝素酶(heparanase,HPSE)在早期自然流产患者与正常早期妊娠妇女绒毛中的表达情况,探讨其在绒毛发育过程中可能发挥的作用。方法:选择2013年12月至2014年12月因早期自然流产行清宫术的患者30例和因计划生育因素自愿行人工流产术的早期妊娠患者30例,将以上两组患者作为研究对象,采集两组患者的绒毛组织,采用免疫组织化学染色法进行HPSE蛋白在两组绒毛组织中的定位及半定量分析。结果:免疫组化染色分析发现,HPSE在早期自然流产组及正常早期妊娠组患者的绒毛小叶合体滋养细胞及细胞滋养细胞中均有表达,HPSE在正常妊娠妇女绒毛中的表达为强阳性,但在早期自然流产患者绒毛中的表达为弱阳性。半定量分析显示HPSE在早期自然流产组的绒毛组织中的阳性表达率为46.7%,正常早期妊娠组为93.3%,组间比较差异有统计学意义(P<0.05)。结论:HPSE蛋白在正常妊娠患者绒毛中的表达高于早期自然流产患者,提示其可能参与绒毛的发育。Objective ： To study the expressions of heparanase （HPSE） in the villi of patients with normal early pregnancy and early spontaneous abortion, and to explore its function in the development of the villi as well. Methods： 30 early spontaneous abortion patients who were performed curettage and 30 normal early pregnancy women who volunteered for artificial abortion because of family planning from December 2013 to December 2014 were chosen as the research objects. Part of the villous tissue of the two groups were collected and stained by immunohistochemical staining method, with HPSE protein in the villi of the two groups analyzed by positioning and the semi - quantitative method. Results： Analysis of immumo- histochemical staining showed that HPSE was expressed in both the villi of leaflets syncytiotrophoblast and cytotrophoblast of the early spontaneous abortion and normal early pregnancy villous tissues, with the expression of HPSE strongly positive in the villi of patients with normal early pregnancy but weakly positive in the villi of patients with early spontaneous abortion. Semi - quantitative analysis showed the HPSE positive expression rate was 46.7 % and 93.3 % respectively in the villus tissue of early spontaneous abortion and normal early pregnancy, the difference being statistically significant （ P 〈 0.05 ）. Conclusion： The expression of HPSE protein was higher in the villi of patients with normal early pregnancy than that in the early spontaneous abortion, suggesting that it may be involved in the development of villi.
