影响肝胆外科患者术后舒适的因素及护理方法

摘　　要：目的:探讨影响肝胆外科患者术后舒适的因素和护理方法。方法:选择2015年1月至2016年12月间收治的肝胆外科手术患者198例作为研究对象,术后5 d内对患者发放调查问卷,记录手术后影响患者舒适度的因素,探讨适合该类患者的护理方法。结果:患者在术后5 d内舒适度评分在3~9分之间,平均为(5.19±1.26)分,术后前3 d舒适度评分比较高,患者的不适程度比较严重,术后第5 d与术后第1 d舒适度评分相比,术后第5 d舒适度评分下降(t=6.719,P<0.05),影响患者舒适度的因素有口干口渴、饥饿、术口疼痛、咳嗽咳痰、胃管引起咽喉疼痛、腹胀、恶心呕吐、尿管引起不适、引流管牵制、担心疾病的预后和对经济担忧等。结论:舒适度是影响肝胆外科患者术后康复的因素之一,护理人员要根据患者术后的不适程度针对性的采取促进舒适的护理措施,提高患者舒适度,促进康复。Objective ： To explore the factors influencing the comfort of patients after hepatobiliary surgery and nursing methods. Methods ： 198 patients after hepatobiliary surgery hospitalized from January 2015 to December 2016 were chosen as the research object. The questionnaires were sent to patients in 5 days, with the factors influencing the patient~ comfort recorded and the nursing methods suitable for the patients discussed. Re- suits ： The postoperative comfort scores of patients within 5 days were between 3 to 9 points, with an average of 5.19 ＋ 1.26 points. The postoper- ative comfort scores within 3 days were higher and the degree of discomfort in patients was more serious. Compared with the postoperative comfort scores on the first day, the scores on the fifth day were lower（ t = 6. 719, P 〈 0.05）. Factors that influence patients＇comfort are thirst, hunger, wound pain, cough ,expectoration, throat pain caused by stomach tubes, abdominal distension, nausea and vomiting, discomfort caused by urinary catheters, containment of drainage tubes and worry about the prognosis of the disease and economic concerns. Conclusion ： Comfort is one of the factors that influence the rehabilitation of patients after hepatobiliary surgery. Nursing staff should take targeted measures to promote nursing comfort according to the degree of discomfort after surgery to improve patients＇comfort and promote rehabilitation.
