低压空气灌肠辅助定位在小儿异位阑尾炎诊治中的应用

摘　　要：目的探讨低压空气灌肠辅助定位在小儿异位阑尾炎诊治中的应用。方法对58例临床症状及血常规等检查符合急性阑尾炎诊断,但右下腹疼痛不明显的患儿(可疑阑尾炎)常规应用电脑遥控灌肠整复仪低压空气灌肠进行术前辅助定位。结果经过辅助定位发现低位(盆腔左侧位)阑尾炎38例,高位(肝下位)阑尾炎18例,左腹部阑尾炎2例。手术时间25~53 min,平均40 min。预后和并发症与非异位阑尾炎无明显差异。结论经术前辅助定位后可做到有目的选择腹部手术切口,避免术中盲目延长切口,这将减轻对患儿身体的创伤,节约手术时间,减少术中副损伤及术后并发症。低压空气灌肠术前辅助定位在小儿异位阑尾炎的诊治中是一种操作简单、经济、方便、实用的方法。Objective To explore the clinical application of low-pressure air enema in the diagnosis and treatment of ectopic appendicitis in children. Methods A total of 58 children had clinical symptoms and blood routine examinations conforming to the diagnosis of acute appendicitis. However, typical right lower quad- rant abdominal pain was absent. Low-pressure air enema was performed under monitoring of computer remoted enema taxis instrument. Results Appendicitis was detected in left abdomen （ n = 2） ,left pelvis （ n = 38 ） and lower liver （n = 18 ）. The average operative duration was 40 （25 -53 ） rain. No significant difference existed between prognosis and complications of normal appendicitis. Conclusions Accurate abdominal incision may be achieved by low-pressure air enema positioning. It helps to avoid the extension of incision, reduce physical trauma and complications and shorten operative duration. Low-pressure air enema is simple, economical, con- venient and practical in the diagnosis and treatment of ectopic appendicitis in children.
