摘　　要：目的:分析和评价社区获得性肺炎(CAP)患者单病种抗菌药物治疗的质控指标,为下一步规范单病种管理提供资料参考。方法:随机抽取2015年度入住呼吸内科患者,第一诊断为CAP且入组临床路径的病例96例,分析抗感染药物治疗的质控指标,分析其病原学诊断、抗感染药物治疗时机、初始抗菌药物的选择、初始治疗效果以及抗感染疗程。结果:抗菌药物使用前的病原学诊断仅为6.25%,96.9%患者能在入院4 h内接受抗菌药物治疗,初始抗感染方案选择针对重症肺炎的百分率为72.92%,全部病例均做了效果评价,抗菌药物治疗的疗程合格率达85.42%。结论:在CAP当中抗感染治疗的单病种管理存在一定的问题,尤其在病原学诊断和初始抗菌药物选择方面;这需要加强培训和管理,提高抗菌药物使用前微生物送检率和初始抗菌药物选择的合理率。Objective： To analyze and evaluate the quality control indexes of of anti-infection medication in single diseases of patients with community acquired pneumonia and to provide a reference tbr the next step of standardized sin- gle disease management. Methods： 96 cases were randomly selected in which first diagnosis was community acquired pneumonia and selected in clinical pathway of Respiratory Medicine Department of Donghua hospital in 2015. The antiinfection treatment was focused on following analysis, such as etiological diagnosis, opportunity of anti infection treatment, selection of initial antimicrobial agents, evaluation of initial therapeutic effect, and anti-infection treatment course. Results： The etiological diagnosis of the use of antibiotics was only 6.25%; 96.9% of the patients were treated with antibiotics within 4 hours of admission, the ratio of initial anti infection to severe pneumonia was 72.9%, all cases were evaluated, and the pass rate of course of antimicrobial thera py was 85.4%. Conclusion： There were some problems in the management of single disease in the anti infection treatment of CAP in our hospital, especially in the aspects of the etiological diagnosis and the selection of initial antimicrobial agents. We need to strengthen the training and management to improve the microbial detection rate before the use of antimicrobial agents and the rational choice of initial antimicrobial agents.
