摘 要:目的:评价抗结核药物与薄芝糖肽对艾滋病患者伴继发性肺结核的疗效及其对免疫指标及病毒载量的影响。方法:选取2016年3月—2016年12月间收治的艾滋病伴继发性肺结核患者60例,采用随机原则分组法将其分为对照组30例和观察组30例;对照组患者给予2HRZE/4HR方案治疗,观察组患者给予2HRZE/4HR与薄芝糖肽治疗,评价两组患者治疗后的总有效率和治疗前后的T淋巴细胞亚群、白细胞计数、病毒载量测得值的变化情况。结果:观察组患者治疗后的总有效率为93.33%高于对照组为60.00%(P<0.05),治疗后CD3(11.75±3.46)、CD4(1.76±0.63)、CD8(1.32±0.45)和CD4/CD8(1.31±0.55)测得值低于对照组(P<0.05),白细胞计数高于对照组(P<0.05),HIV-RNA测得值为(4.26±0.80)×102U/m L低于对照组为(4.81±0.60)×102U/m L(P<0.05)。结论:采用2HRZE/4HR与薄芝糖肽的治疗方案治疗艾滋病伴继发性肺结核感染患者,其效果远优于单用2HRZE/4HR的疗效,改善了HIV-RNA和T淋巴细胞亚群,提高了抗结核分枝杆菌的疗效。Objective: To evaluate the efficacy and influence on immunity and virus contents in HIV/AIDS patients with secondary tuberculosis (TB) by using antituberculosis drugs and Bozhi glycopeptide. Methods: From March 2016 to December 2016 among patients with secondary tuberculosis in 60 cases of A1DS, using the principle of random grouping method dividing into the control group for 30 cases and 30 cases in the observation group; the control group were treated with 2HRZE/4HR regimen, the observation group were treated with 2HRZE/4HR Bozhi glycopeptide; evaluation of the two groups after treatment as follows: the total change of T lymphocyte subsets, white blood cell count, viral load and efficiency before and after the treatment of the measured value. Results: In the observation group after treatment, the total effective rate was 93.33% higher than that of the control group (P〈0.05); 60.00% patients in the observation group after treatment of CD3(1 1.75±3.46)102 U/mL, CD4(1.76±0.63)102 U/mL, CD8(1.32±0.45)102 U/mL and CD4/ CD8 (1.31±0.55)102 U/mL the measured value is lower than that of the control group (P〈0.05), white cell count higher than that of the control group (P〈0.05); the patients in the observation group HIV-RNA measured value is (4.26~0.80) 10-~ U/mL is lower than that of the control group (4.81 ~0.60)102 U/mL (P〈0.05). Conclusion: In the treatment of AIDS 2HRZE/4HR and Bozhi glycopeptide with secondary TB patients, the effect is far better effect than the pure 2HRZE/ 4HR, improving HIV-RNA and T lymphocyte subsets, and the curative effect of the anti mycobacterium tuberculosis is also improved.
