腹腔镜下完全腹膜外途径一期手术治疗非腹腔型隐睾伴腹股沟斜疝

摘　　要：目的探讨腹腔镜下完全腹膜外途径一期行睾丸下降固定及疝修补术治疗非腹腔型隐睾伴腹股沟斜疝的可行性。方法运用腹腔镜下完全腹膜外途径一期行隐睾下降固定及疝修补术治疗非腹腔型隐睾合并斜疝的患者,观察手术时间、睾丸下降情况、并发症等并总结初步的临床经验。结果 2012年6月至2015年12月期间我院共5例患者,儿童2例,成人3例,均为非腹腔型隐睾合并腹股沟斜疝,其中1例成人为双侧。5例患者均顺利完成手术,手术时间平均108 min,术中平均出血量约25.8 ml。随访5~25个月(平均16个月),术后无切口感染、睾丸萎缩或回缩、腹股沟斜疝复发、粘连性肠梗阻及脐疝。结论腹腔镜下完全腹膜外途径一期行隐睾下降固定及疝修补术治疗非腹腔型隐睾合并斜疝是可行的、安全的。Objective To explore the feasibility of laparoscopic total extraperitoneal approach for descending fixation of cryptorchidism and herniorrhaphy in the treatment of non-abdominal cryptorchidism with oblique inguinal hernia. Methods A total of 5 cases of non-peritoneal cryptorchidism with oblique inguinal hernia were treated with laparoscopic total extraperitoneal approach for one-stage descending fixation of cryptorchidism and hernia repair. The operation time, testicular descent, complications and so on were observed and the preliminary clinical experiences were summarized. Results Between June 2012 and December 2015, there were 5 patients（2 children and 3 adults） in our hospital who had non-abdominal cryptorchidism with oblique inguinal hernia, of which 1 adult was bilateral. Five cases were successfully completed the operation. The average operation time was 108 min and the average intraoperative blood loss was about 25.8 ml. The follow up was 5 to 25 months（mean 16 months）. Incision infection, testicular atrophy or retraction, inguinal hernia recurrence, adhesive intestinal obstruction or umbilical hernia and other surgical complications were not occurred. Conclusion The total extraperitoneal laparoscopic orchidopexy and hernia repair in the treatment of non abdominal cryptorchidism with oblique inguinal hernia is feasible and safe.
