摘 要:目的:探讨复方麝香注射液联合依达拉奉对颅脑外伤患者凝血功能、神经功能的影响及安全性。方法:选取我院2015年4月-2016年4月收治的颅脑外伤患者84例,按照随机数字表法分为观察组与对照组,各42例。对照组患者给予依达拉奉注射液30 mg加入0.9%氯化钠注射液100 m L中,bid,ivgtt;观察组在对照组基础上加用复方麝香注射液20 m L加入0.9%氯化钠注射液250 m L中,qd,ivgtt。两组患者均治疗4周。比较两组患者治疗前后神经功能缺损程度(NIHSS)评分、格拉斯昏迷分级(GCS)评分、血清神经元特异性烯醇化酶(NSE)水平、C反应蛋白(CRP)水平、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、D-二聚体水平、格拉斯预后分级(GOS)评分及不良反应发生情况。结果:治疗后,两组患者NIHSS评分均较治疗前显著降低,且观察组显著低于对照组,差异有统计学意义(P<0.05);两组患者GCS评分均较治疗前显著增高,且观察组显著高于对照组,差异有统计学意义(P<0.05);两组患者血清NSE、CRP、PT、APTT和D-二聚体水平均较治疗前显著降低,且观察组显著低于对照组,差异有统计学意义(P<0.05);观察组患者GOS评分显著高于对照组,差异有统计学意义(P<0.05)。两组患者治疗期间未见明显不良反应发生。结论:复方麝香注射液联合依达拉奉可改善颅脑外伤患者凝血功能和神经功能,且可减轻患者颅脑损伤严重程度,改善预后。OBJECTIVE: To investigate the effects and safety of Compound musk injection combined with edaravone on coagulation and neurological function of patients with traumatic brain injury. METHODS: A total of 84 patients with traumatic brain injury in our hospital during Apr. 2015 to Apr. 2016 were divided into observation group and control group, with 42 cases in each group. Control group was given Edaravone injection 30 mg+0.9% sodium chloride injection 100 mL, bid, ivgtt; observation group was additionally given Compound musk injection 20 mL+0.9% sodium chloride injection 250 mL, qd, ivgtt. Both groups were treated for 4 weeks. NIHSS score, GCS score, the levels of NSE, CRP, PT, APTT and D-dimer, GOS score, the occurrence of ADR were compared between 2 groups before and after treatment. RESULTS: After treatment, NIHSS score of 2 groups decreased significantly, compared to before treatment; the observation group was significantly lower than the control group, with statistical significance (P〈0.05). GCS score of 2 groups increased significantly, compared to before treatment; and the observation group was significantly higher than the control group, with statistical significance (P〈0.05). The serum levels of NSE, CRP, PT, APTT and D-dimer in 2 groups decreased significantly, compared to before treatment; and the observation group was significantly lower than the control group, with statistical significance (P〈0.05). GOS score of observation group was significantly higher than that of control group, with statistical significance (P〈0.05). No obvious adverse reaction was found in 2 groups during treatment. CONCLUSIONS: Compound musk injection combined with edaravone can improve coagulation and neurological function of patients with traumatic brain injury, relieve traumatic brain injury and improve prognosis.
