右美托咪定在老年手术患者麻醉诱导不同阶段中的应用效果

摘　　要：目的探讨右美托咪定在老年手术患者麻醉诱导不同阶段中的应用效果。方法选取金坛区中医医院2015年3月—2016年2月行手术的老年患者86例,随机分为对照组和观察组,各43例。患者于术前均行全身麻醉,对照组患者于麻醉诱导后给予右美托咪定,观察组患者于麻醉诱导前给予右美托咪定。记录比较两组患者于麻醉前(T_0)、诱导前(T_1)、气管插管后1 min(T_2)、气管插管后5 min(T_3)、气管插管后10 min(T_4)的血压和心率,并观察比较两组患者不良反应发生情况。结果不同时间点两组患者舒张压、心率比较,差异无统计学意义(P>0.05);T_0、T_1、T_3、T_4时,两组患者收缩压比较,差异无统计学意义(P>0.05);T_2时,观察组患者收缩压低于对照组(P<0.05)。观察组患者不良反应发生率低于对照组(P<0.05)。结论在老年手术患者麻醉诱导前应用右美托咪定,可有效稳定血流动力学,减少不良反应发生。Objective To investigate the application effect of dexmedetomidine during different induction of anesthesia induction in elderly patients With surgery. Methods A total of 86 cases of elderly patients with surgery were selected from March 2015 to February 2016 in Jintan Hospital of Traditional Chinese Medicine in Changzhou City, which were randomly divid- ed into control group and observation group, 43 cases in each group. Both groups were given general anesthesia, control group was given dexmedetomidine after anesthesia induction, observation group was given dexmedetomidine before anesthesia induction. The blood pressure and heart rate at the time of T0, T1, T2 , T3 , T4 were compared between the two groups, and the incidence of adverse reactions were observed between the two groups. Results Before and after anesthesia, no significant difference of diastolic blood pressure or heart rate was found between the two groups （P 〉 0, 05） ; at the time of T0, T1, T3, T4, no significant difference of systemic blood pressure was found between the two groups （P 〉 0. 05 ） ; at the time of T2, the systemic blood pressure of observation group was lower than control group （ P 〈 0. 05 ）. The observation group of the adverse reac- tions rate was lower than control group （P 〈 0.05）. Conclusion The administration of dexmedetomidine in elderly patients before anesthesia induction can effectively stabilize the hemodynamics of patients, and reduce the adverse reactions.
