摘  要:【目的】探究3种体液microRNAs联合血清硅含量检测早期矽肺的指示意义。【方法】采用完全随机设计方法选取早期矽肺患者,并排除肺结核活期、哮喘、慢性阻塞性肺疾病等肺部急慢性疾病;选择年龄相近、没有基础疾病的健康人群作为对照组。通过人外周血淋巴细胞分离液分离外周血淋巴细胞,采用Exo Quick TM试剂盒提取人血清exosome,应用原子吸收法测定血清硅含量,应用RT-q PCR定量检测microRNAs表达水平,通过受试者工作特征曲线(receiver operating characteristic curve,ROC)判定microRNAs联合血清硅含量指示早期矽肺的灵敏度和特异度。【结果】矽肺各期次同对照组血清硅元素含量比较,对照组为(3.02±1.57)μg/ml,0+期矽肺组为(3.86±1.29)μg/ml,Ⅰ期矽肺组为(4.61±1.96)μg/ml,矽肺组相比正常组具有增高的趋势。矽肺患者血清、外周血淋巴细胞、血清exosome mi R-16-5p的相对表达量为(3.0105±1.2220)、(0.0321±0.0154)、(4.7309±1.1193),let-7d-5p相对表达量为(0.1652±0.0576)、(0.0049±0.0018)、(0.4026±0.1664)。血清硅联合血清microRNAs检测ROC曲线下面积比最大达到94.4%,敏感性为100%,特异度为66.7%;血清硅联合外周血淋巴细胞microRNAs检测的ROC曲线下面积比为77.8%,敏感性为83.3%,特异度为66.7%;血清硅联合血清exosome microRNAs检测曲线下面积比为77.8%,敏感性为83.3%,特异度为66.7%。【结论】血清硅联合血清mi R-16-5p、let-7d-5p检测具有较高的矽肺诊断指示意义。【Objective】To explore the indication significance of microRNAs combined with serum silicon content in three kinds of body fluids in the detection of early silicosis.【Methods】Based on a completely randomized design method, the patients with early silicosis were selected and excluded from active pulmonary tuberculosis, asthma, chronic obstructive pulmonary diseases and other acute and chronic lung diseases. A healthy population with similar age and no underlying diseases was selected as the control group. Peripheral blood lymphocytes were isolated by human peripheral blood lymphocyte separation fluid. The human serum exosomes were extracted by the Exo Quick TM kit. The serum silicon content was determined by atomic absorption spectrometry. The expression levels of microRNAs were quantitatively detected by RT-q PCR. The receiver operating characteristic(ROC) curve was used to determine the sensitivity and specificity of microRNAs combined with serum silicon content in the detection of early silicosis. 【Results】 Compared with the control group(3.02 ± 1.57) μg/ml, the serum levels of silicon in the silicosis groups of stage 0+ and Ⅰ were(3.86 ± 1.29)μg/ml and(4.61 ±1.96)μg/ml. There was an upward trend in silicosis groups. The relative expression levels of mi R-16-5p in serum, peripheral blood lymphocytes and serum exosomes of silicosis patients were(3.0105 ± 1.2220),(0.0321 ± 0.0154) and(4.7309 ± 1.1193). The relative expression levels of let-7d-5p were(0.1652 ± 0.0576),(0.0049 ± 0.0018) and(0.4026 ± 0.1664). The area under the ROC curve of serum silicon combined with serum microRNAs was up to 94.4%, and the sensitivity and specificity were respectively 100% and 66.7%.The area under the ROC curve of serum silicon element combined with peripheral blood lymphocyte microRNAs was 77.8%, and the sensitivity and specificity were respectively 83.3% and 66.7%.
