摘 要:目的了解福州市急性胃肠炎暴发疫情患者诺如病毒病原学特征及流行特点。方法收集2015-2017年福州市疑似诺如病毒引起的急性胃肠炎暴发疫情患者的肛拭子或呕吐物标本,应用实时荧光定量聚合酶链反应(RT-PCR)筛查诺如病毒,阳性标本应用反转录聚合酶链反应扩增部分VP1区基因并测序,使用诺如病毒基因在线分析工具以及种系进化分析方法了解诺如病毒基因型和亚型的分布。结果在对21起疫情158份患者标本的核酸检测中,实时荧光RT-PCR法共检出GⅡ型89份(56.33%),GⅠ型9份(5.70%)。在随机抽取的有代表性的21份阳性标本中,共获得13份部分VP1区基因序列,其中8份GⅡ.2亚型,2份GⅡ.3亚型,2份GⅡ.17亚型,1份GⅠ.6亚型。结论 2015-2017年福州市引起急性胃肠炎暴发疫情的诺如病毒病原学基因亚型优势毒株为GⅡ.2亚型,且GⅡ.17和GⅡ.2亚型为福州首次发现,其余均为本地流行的型别。Objective To investigate the etiological and epidemiological characteristics of acute gastroenteritis outbreaks caused by norovirus in the city of Fuzhou. Methods Samples of anal swabs and vomitus from patients of acute gastroenteritis caused by susupected norovirus outbreaks between 2015 and 2017 in Fuzhou were collected. Real-time RT-PCR was used to screen and identify the positive norovirus specimens. In positive samples, some of the viral VP1 gene fragments were further am- plified by reverse-transcription PCR (RT-PCR) and then sequenced. Online norovirus gene analytical tools and phylogenetic a- nalysis were used to identify the virus genogroups and genotypes. Results We examined 158 specimens of patients from 21 norovir- us outbreaks, 89 of which were identified as G Ⅱ geneogroup norovirus(56.33 % ), and 9 as G I geneogroup (5.70%). Moreo- ver, we obtained thirteen parts of VP1 DNA sequences from a total of twenty-one representative virus-positive specimens. The sequences showed that eight of them were G Ⅱ. 2 strain, two G Ⅱ.3 strain, two GⅡ II. 17 strain, and one G I. 6 strain. Conclu- sion The majority of norovirus subtypes that caused the outbreaks of the epidemic and acute gastroenteritis between 2015 and 2017 in Fuzhou were G Ⅱ.2 strain. Importantly, both GⅡ.2 and G ⅡI1.17 strains were identified for the first time. The rest of them were strains locally identified before.
