摘 要:目的探讨多层螺旋CT(MSCT)扫描与CT血管造影(CTA)技术对肾脏病变及肾血管损伤的临床应用价值分析。方法选取2013年1月-2014年8月我院因肾脏疾病进行MSCT扫描与CTA检查的患者94例。整理94例患者MSCT扫描图像与CTA重建图像,由两名经验丰富的放射科诊断医师共同评估观察MSCT扫描图像与MSCTA重建图像,对肾脏及血管病变的检出个数与扫描检查优势进行分析。结果 MSCT扫描对94例患者检出病灶结节共58例,其中肾实质病变36例,囊性病变9例,肾畸形者12例,肾动脉血栓1例;CTA技术图像对94例患者检查出病灶结节共73例,其中肾实质病变37例,囊性病变9例,肾畸形12例,肾血管病变15例。MSCT对肾实质病变检出例数较多,对血管病变检出例数劣于CTA技术。CTA技术图像对94例患者病灶检出例数较MSCT平扫多(χ~2=22.267,P<0.05)。结论 MSCT扫描图像对肾实质结构变化显示突出,CTA各项后处理技术能直观、全面的准确显示病变区,在多角度体现肾脏与血管的空间解剖关系。MSCTA有利于影像医生对病变部位进行精确定位显示,准确掌握患者肾血管解剖走形,为临床提供重要影像学资料,指导临床医生制定正确的治疗方案。Objective To investigate the clinical value of MSCT scanning and CTA in the diagnosis of renal lesions and renal vascular injury. Methods A total of 94 patients subjected to MSCT scan and CTA examination in our hospital between Janu- ary 2013 to August 2014 were enrolled in this study. The images of MSCT and CTA, reconstructed from these patients, were eval- uated by two experienced radiologists to analyze the number of renal and vascular lesions and the advantages of scanning. Results MSCT scans detected a total of 58 cases of nodular lesions in these patients, including 36 cases of renal parenchymal disease ,9 cases of cystic lesions, 12 cases of kidney malformations, and 1 case of renal artery thrombosis. CTA results spotted 73 nodules in these 94 patients,including 37 lesions of renal parenchyma,9 cystic lesions, 12 renal abnormalities and 15 renal vascular lesions. MSCT detected more renal parenchymal lesions than CTA did, but CTA detected more lesions than MSCT (χ2= 22. 267,P〈 0. 05 ). Conclusion MSCT scan images of renal parenchymal structural changes show that CTA post-processing technology can in- tuitively, comprehensively and accurately display lesions, and reflect the anatomical relationships between the kidney and blood vessels from multiple angles. MSCTA can help doctors to accurately locate the lesion, better understand renal vascular anatomy, providing important imaging data for clinicians to select the right treatment.
