超声引导颈内静脉置管术对肿瘤患者导管相关性感染的影响

摘　　要：目的探讨超声引导下行颈内静脉精确置管的优势及其对肿瘤患者导管相关性感染的影响。方法将2014年1月-2015年12月的346例实施颈内静脉穿刺置管术的住院肿瘤患者随机分为对照组和超声组,分别采用盲穿法和超声引导下进行颈内静脉置管操作,比较2组一次穿刺成功率、总成功率、平均完成置管操作的时间、导管相关性感染发生率以及平均导管留置时间。结果对照组与超声组一次穿刺成功率分别为87.3%和97.7%,总成功率分别为96.5%和100%,平均完成置管操作的时间分别为(9.62±2.33)min和(8.48±1.45)min,导管相关性感染率分别9.6%和3.5%,差异均有统计学意义(P<0.05)。结论超声引导颈内静脉精确置管能有效提高穿刺成功率,减少肿瘤患者导管相关性感染的发生。Objective To investigate ultrasound-guided internal jugular vein accurate catheterization to prevent catheter-related infections in cancer patients. Methods A prospective study was selected in hospitalized cancer patients who receive internal jugular vein catheterization from January 2014 to December 2015. 346 cases were randomly divided into ultrasound and control group,the control group receive blind method internal jugular vein catheterization,ultrasound group receive ultrasound-guided internal jugular vein catheterization. The success rate of first puncture,the total success rate,the mean operating time,the mean retention time and the rate of catheter-related infections were compared. Results The success rate of first puncture of contrl group and ultrasound group were 87. 3% and 97. 7% respectively. The mean operating time of contrl group and ultrasound group were（ 9. 62 ± 2. 33） min and（ 8. 48 ± 1. 45） min,the rate of catheter-related infections of contrl group and ultrasound group were 9. 6% and 3. 5%,all the difference were statistically significant（ P〈0. 05）. Conclusion Ultrasound-guided internal jugular vein accurate catheterization can effectively improve the puncture success rate and reduce catheter-related infections in cancer patients.
