摘 要:目的:探讨高渗葡萄糖(50%葡萄糖)、地塞米松及碘海醇联合应用在婴儿排泄性尿路造影中的效果。方法:行排泄性尿路造影术、且肾功能正常的婴儿患儿60例,随机分成对照组和实验组;对照组患儿采用单纯注入碘海醇进行造影,实验组患儿在对照组的基础上于注药前30 min给予地塞米松注入,30 min后给予1/2碘海醇剂量的高渗葡萄糖(50%葡萄糖)溶液联合用药进行造影(并加温至30℃左右后注药);比较两组患儿的肾脏显影、输尿管显影、图像质量及不良反应数据。结果:实验组患儿注药7 min后,肾盂肾盏及输尿管清楚显影;对照组注药7 min后,肾盂轻度显影,膀胱充盈不佳;实验组患儿在7 min之内的肾脏显影清楚、输尿管显影清楚的例数均高于对照组患儿,差异有统计学意义(P<0.05);实验组患儿的甲级图像数据高于对照组,丙级图像数据低于对照组患儿(P<0.05),乙级图像数据与对照组患儿比较,差异无统计学意义(P>0.05);两组患儿的不良反应比较差异无统计学意义(P>0.05)。结论:高渗葡萄糖、地塞米松及碘海醇联合应用能够提升婴儿排泄性尿路造影中的图像质量,安全性较高。Objective: To study the clinical effect of hypertonic glucose (50% glucose), dexametha- sone and iohexol in combination with the excretory urography of Infant. Methods: 60 cases of infants underwent the excretory urography and normal renal function from January 2015 to October 2016 in the hospital, and the patients were randomly divided into experimental group and control group. The con- trol group was conducted by simple injection of iohexol radiography; the experimental group was con- ducted by the injection of hexadecadrol before 30 minutes, then adding the drug combination of hyper- tonic glucose (50% glucose) of l/2 iohexol dose to conduct radiography, which was heated to 30 de- grees Celsius after the injection. The data of renal and ureteral development status, image quality and adverse reaction was compared between the two groups. Results: After 7 minutes of injection in the ex- perimental group, pelvicaliceal and ureter were clearly developed. After 7 minutes of injection in the control group, pelvis was slightly developed and filling of bladder was not good. In 7 minutes of injec- tion in the experimental group, kidney was clearly developed and the number of cases about the clear ureter development was higher than in the control group, so the differences were statistically significant ( P 〈 0.05 ). The image data A of experimental group was higher than the control group ; the image da- ta C of experimental group was lower than the control group (P 〈 0.05 ). As image data B was com- pared with the control group, the differences were not statistically significant (P 〉 0.05 ). The differ- ence of bad reaction in the two groups was not statistically significant ( P 〉 0.05 ). Conclusion: The combined application of hypertonie glucose (50% glucose) , dexamethasone and iohexol can improve the image quality of radiography of excretory urography in infants.
