摘 要:目的对广西2014年第三批县级公立医院综合改革试点县的县级公立综合医院的医疗服务质量进行评价,探索多种评价方法组合使用的可行性。方法采用自编调查问卷调查35家广西县级公立综合医院,然后通过查阅资料文献选取3个维度13个指标作为医疗质量评价指标体系,在主成分分析法提取主成分并计算出各主成分权重系数的基础上,组合运用加权Topsis法和RSR法对医疗质量进行评价。结果评价得分排序显示,Topsis法和RSR法评价结果一致(Spearman相关系数0.9,P<0.001),RSR分档结果显示,优、良、中、差分别占17.1%、34.3%、34.3%、14.3%。结论在主成分分析法的基础上组合使用Topsis法和RSR法评价医疗质量具有科学性和可行性。广西第三批县级公立医院改革试点的35家县级公立综合医院医疗水平、设备较落后,医疗质量相对较高的几家医院可能和当地总人口数有关。Objective To evaluate the medical quality of Guangxi county level public general hospitals from the third comprehensive reform test counties in 2014 and explore the feasibility of combination of weighted Topsis method and weighted RSR. Methods Self-designed questionnaires were used to investigate the quality of county level public general hospitals. Three dimensions including 13 indexes were created according to literature survey. Weighted Topsis and weighted RSR methods were usedon the basis of principal component analysis. Results The results of Topsis and RSR were consistent (the Spearman correlation coefficient was 0.9, P〈0.001). RSR classifying results were 17.1%(excellent), 34.3%(good), 34.3% (middle), 14.3%(poor). Conclusion On the basis of principal component analysis, the combination of Topsis and RSR method is reliable and feasible. These hospitals located in poor areas with poor medical facilities. Better medical quality may be related to larger total population.
