复方丹参注射液对于新生儿缺氧缺血性脑病的疗效以及血清Tau蛋白对其评价的临床意义

摘　　要：目的:探讨新生儿缺氧缺血性脑病(HIE)患儿血清Tau蛋白水平的变化,以及采用复方丹参注射液治疗HIE的过程中Tau水平监测对于疗效监测的临床价值。方法:选择新生儿HIE 60例,随机分为观察组和对照组,对照组采用常规治疗,观察组在常规治疗的基础上采用复方丹参注射液治疗,于治疗前、治疗第3、7、10、14、21、28天分别进行NBNA评分及血清Tau蛋白水平检测,对比两组之间的治疗效果、不同治疗时期Tau检测结果,并分析不同治疗时期Tau检测结果与NBNA评分的相关性。结果:观察组的患儿在症状缓解、惊厥消失、住院病程等方面的指标均明显优于对照组患儿,获得较为明显的治疗效果。治疗前及治疗第3天两组患儿的Tau检测结果无统计学差异(P>0.05),自治疗第3天起至疗程结束日,观察组的Tau均显著低于对照组(P<0.01);Tau浓度与NBNA评分呈现出显著负相关(P<0.05)。结论:Tau水平测定可作为协助判断HIE患儿的脑损伤程度及评价复方丹参注射液治疗效果的临床指标。Objective：To investigate the changes of serum Tau protein levels in children with hypoxic-ischemic encephalopathy（HIE） and the monitoring of Tau levels in the treatment of HIE with Compound Salvia Miltiorrhiza Injection.Methods：Sixty patients with neonatal hypoxic-ischemic encephalopathy（HIE） were randomly divided into observation group and control group.The control group was treated with conventional therapy.The observation group was treated with Compound Salvia Miltiorrhiza Injection on the basis of routine treatment.Respectively on the 3rd,7th,10 th,14th,21 st and 28 th day NBNA score and serum Tau protein levels were measured.Tau test results in different treatment periods between the two groups and the correlation with the NBNA score were compared.Results：The indexes of the observation group were significantly better than those of the control group in symptom relief,convulsions disappearance,hospital course and so on,and the obvious therapeutic effect was obtained.There was no significant difference in Tau between the two groups before and 3 days after treatment（P〉 0.05）.The Tau of the observation group was significantly lower than that of the control group（P〈0.05）.Tau concentration showed a significant negative correlation with NBNA score（P〈0.05）.Conclusion：Tau level can be used as a clinical index to judge the degree of brain injury in children with HIE and evaluate the therapeutic effect of Compound Salvia Miltiorrhiza Injection.
