中晚期妊娠胎儿丢失的病因分析

摘　　要：目的分析中晚期妊娠胎儿丢失的病因,为提高围产保健水平,减少胎儿丢失提供依据。方法收集2015年1月1日—12月31日在福建省妇幼保健院建档产前检查、终止妊娠的孕中晚期胎儿丢失病例,分析胎儿丢失病因。结果 (1)胎儿丢失543例,胎儿丢失病因依次为胎儿因素43.83%、母体因素24.68%、胎盘因素15.10%、不明原因9.58%、脐带因素6.81%。(2)胎儿因素中71.01%发生于中孕期,母体因素、胎盘因素分布于中孕期的各占88.81%、85.37%。(3)脐带因素64.86%分布于晚孕期,是晚期妊娠胎儿丢失的重要原因。结论重视中孕期保健可减少胎儿丢失,避免严重畸形胎儿进入围产期,降低围产儿死亡率。妊娠晚期提高脐带异常的产前诊断率,关注胎动异常情况,加强胎儿监护,可降低死胎发生率。Objective To analyze causes of fetal loss in second and third trimester, in order to provide basis for improving the level of perinatal care and reducing fetal loss. Methods The data of puerpera who had terminated pregnancy during the second and third trimester was conducted based on the filling registrations for perinatal examinations in Fujian Provincial Maternity and Children Hospital from January 1, 2015 to December 31, 2015 were collected to analyze the causes of fetal loss. Results（1）There were 543 puerpera lost fetal, the causes of fetal loss were fetal factor 43.83%, maternal factor 24.68%, placental factor 15.10%, unknown reason 9.58% and umbilical cord factor 6.81%.（2）The fetal loss caused by fetal factor was 71.01% happened in second trimester, maternal factor and placental factor accounted for 88.81% and 85.37% respectively in second trimester.（3）Umbilical cord factor was an important reason for fetal loss in third trimester, accounted for 64.86%. Conclusion Attention to the care during second trimester can reduce fetal loss, avoid severe congenital malformation into perinatal period, in order to reduce perinatal mortality.To improve prenatal diagnosis rate of umbilical cord abnormalities, pay attention to abnormal fetal movement in third trimester, intensify fetal monitoring in order to reduce the incidence rate of stillbirth.
