肌肉注射胃复安预处理对食管心房调搏检查结果阳性率及假阳性率的影响

摘　　要：目的探讨肌肉注射胃复安预处理对食管心房调搏检查结果阳性率及假阳性率的影响。方法拟行食管心房调搏检查的患者120例,随机分为试验组(n=63)和对照组(n=57),试验组食管心房调搏前20 min肌注10 mg胃复安注射液,对照组肌注等体积生理盐水,比较两组食管心房调搏检查结果的阳性率和假阳性率。结果两组患者窦房结功能不良阳性率、房室折返性心动过速阳性率比较,差异无统计学意义(P>0.05)。试验组房室结双径路阳性率、房室结折返性心动过速(AVNRT)阳性率和总阳性率高于对照组;可疑窦房结功能不良假阳性率、房室前传功能障碍假阳性率低于试验组(均P<0.05)。结论肌肉注射胃复安预处理能提高食管心房调搏的房室结双径路和AVNRT诊断阳性率,降低可疑窦房结功能不良假阳性率和房室传导功能障碍假阳性率,值得在临床推广。Objective To investigate the effect of pre-conditioning with metoclopramide intramuscular injection on the positive rate and false positive rate of transesophageal atrial pacing. Methods A total of 120 patients who scheduled for transesophageal atrial pacing were randomly divided into experimental group（n = 63） and control group（n = 57）. The experimental group was given intramuscular injection with 10 mg of metoclopramide at 20 minutes before transesophageal atrial pacing,while the control group was given the same volume of normal saline. The positive rate and false positive rate of transesophageal atrial pacing were compared between the two groups. Results There was no statistically significant difference in the positive rate of sinus node dysfunction or the positive rate of atrioventricular reentrant tachycardia between the two groups（P〉0. 05）. The positive rate of dual atrioventricular node pathways,the positive rate of atrioventricular nodal reentrant tachycardia（AVNRT） and the total positive rate of the experimental group were higher than those of the control group,the false positive rates of suspected sinus node dysfunction and atrioventricular episode dysfunction of the experimental group were lower than those of the control group（all P〈0. 05）. Conclusion Pre-conditioning with metoclopramide intramuscular injection can improve the diagnostic positive rates of dual atrioventricular node pathways and AVNRT in transesophageal atrial pacing,and reduce the false positive rates of suspected sinus node dysfunction and atrioventricular episode dysfunction,which is worth popularizing in clinical practice.
