摘　　要：目的调查某院呼吸内科住院患者抗真菌药物使用情况,并对抗真菌药的临床使用作出评价。方法采用回顾性调查方法,调查2015年9月1日—12月31日出院的使用过抗真菌药物的呼吸内科患者的病历,以《肺真菌病诊断和治疗专家共识》、《侵袭性肺部真菌感染的诊断标准与治疗原则(草案)》、国家抗微生物治疗指南和药品说明书为主要评价标准,对抗真菌药物的临床使用合理性进行评价。结果在收集的136例病历中,用药不合理的病历占60例,其中包括无指征用药32例,品种选用不当34例,首次给药剂量未负荷34例。结论抗真菌药物不合理应用情况常见,主要表现在无指征用药、品种选择不当以及用法存在错误等方面,抗真菌药物使用的管理和培训有待加强。Objective To investigate the usage of antifungal agents among patients hospitalized in the respiratory medicine department of a hospital and evaluate its rationality. Methods The retrospective analysis method was used to investigate the usage of antifungal agents among the discharged patients in the respiratory medicine department of a hospital from September 1 st, 2015 to December 31st, 2015. The evaluation criteria were based on ＂The diagnosis and expert consensus of pulmonary mycosis＂, ＂Diagnostic criteria and principles of treatment of invasive pulmonary fungal infection （draft）＂, ＂The national guide to antimicrobial therapy＂, and ＂Package insert＂. Results 136 patients were included in the analysis. 60 cases of irrational drug use were founded. The main problems were medicines used without indications （32 cases）, improper selection （34 cases）, and no loading dose for the initiation （34 cases）. Conclusion Irrational prescriptions of antifungal drugs in respiratory medicine were really common in the department of respiratory, and the irrational prescriptions mainly included the indication of medicine, drug selection, and dosage of administration. It is necessary to strengthen the control and train the staffs for the standard use of antifungal agents to improve the safety and rationality of the clinical use of drugs.
