贺氏针灸三通法治疗Wallenberg综合征14例临床观察

摘　　要：目的观察贺氏针灸三通法治疗Wallenberg综合征的临床疗效。方法将28例急性延髓背外侧梗死的Wallenberg综合征患者随机分为贺氏三通法组和常规针灸组,每组14例。贺氏三通法组采用贺氏三通法中的微通法、温通法、强通法联合治疗,常规针灸组采用常规针刺取穴治疗,两组均每周治疗5次,共治疗4周。两组患者治疗前后及随访时(治疗结束20周后)分别采用改良Rankin量表(MRS)、Barthel指数(BI)、脑卒中专门化生活质量量表(SS-QOL)、洼田饮水试验、标准吞咽功能评价量表(SSA)评分评价患者神经功能与吞咽功能。结果治疗4周后两组患者各指标评分与治疗前比较差异均无统计学意义(P>0.05),且两组间比较差异也无统计学意义(P>0.05)。随访时两组MRS、BI、SS-QOL评分以及贺氏三通法组洼田饮水试验、SSA评分均较本组治疗前显著改善(P<0.05或P<0.01),且贺氏三通法组洼田饮水试验、SSA评分优于常规针刺组(P<0.01)。结论贺氏针灸三通法可有效改善Wallenberg综合征患者吞咽功能,且远期疗效优于常规针刺。Objective To observe the clinical efficacy of HE＇s three dredging needle methods in treating Wallenberg syndrome. Methods Twenty eight acute dorsolateral medullary infarction patients with Wallenberg syndrome were randomly divided into HE＇ s three removing obstruction methods group and routine acupuncture group,with 14 cases in each group. HE＇s three dredging needle methods group was given combined treatment of micro-removing obstruction method,warm-removing obstruction method and strong-removing obstruction method. Routine acupuncture group was given routine acupuncture treatment. The two groups were both treated 5 times each week for 4 weeks.Modified Rankin Scale（ MRS）,the Barthel index of ADL（ BI）,Stroke Specific Quality of Life Scale（ SS-QOL）,water swallow test,and Standardized Swallowing Assessment Scale（ SSA） were measured in the two groups to evaluate neurologic function and swallowing function of patients before and after treatment and at the time of followed up（ 20 weeks after treatment）. Results There was no significant difference in all indexes in the two groups between before treatment and after 4 weeks of treatment（ P〉 0. 05）,and there was also no significant difference in those indexes between the two groups（ P 〉0. 05）. At the time of followed up,MRS,BI and SS-QOL scores in the two groups,as well as the results of water swallow test and SSA scores in HE＇s three removing obstruction methods group were all observably improved compared with those before treatment（ P 〈0. 05 or P 〈0. 01）,and the results of water swallow test and SSA scores in HE＇s three removing obstruction methods group were better than those in routine acupuncture group（ P〈 0. 01）. Conclusion HE＇ s three dredging methods could effectively improve swallowing function of Wallenberg syndrome patients,and the long-term effect of HE＇s three dredging methods seems better than that of routine acupuncture.
