摘 要:目的探究肾脑复元汤联合人脐带间充质干细胞(h UC-MSCs)移植治疗缺血性中风的可能机制。方法将75只大鼠随机分为假手术组、模型组、中药组、干细胞组、联合组各15只,除假手术组外其余各组建立大脑中动脉缺血再灌注模型,造模后第1天中药组和联合组给予肾脑复元汤11.6 g/kg灌胃,假手术组、模型组、干细胞组予10 ml/kg蒸馏水灌胃,每天1次,共15天;干细胞组和联合组造模后第1天尾静脉缓慢注射1×106个h UC-MSCs。分别于术前24 h、术后2 h及术后第4、8、15天进行神经功能评分;分别于第4、8、15天时采用TUNEL染色标记凋亡细胞,检测血清肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)、白细胞介素6(IL-6)、白细胞介素10(IL-10)的表达。结果与模型组相比,中药组(第8、15天)、干细胞组及联合组(第4、8、15天)大鼠神经功能评分降低,凋亡细胞数减少,IL-1β、IL-6表达降低,IL-10表达升高(P<0.05或P<0.01);与中药组比较,联合组第4、8天TNF-α表达降低(P<0.05);与中药组及干细胞组比较,联合组第8天时IL-1β、IL-6表达降低,第8、15天时神经功能评分降低、凋亡细胞数减少、IL-10表达升高(P<0.05)。结论肾脑复元汤联合h UC-MSCs移植可促进大脑中动脉缺血再灌注大鼠神经功能修复,其机制可能与调节炎症反应有关。Objective To explore the potential mechanism of Shennao Fuyuan Decoction( 肾脑复元汤,SFD)combined with human umbilical cord mesenchymal stem cell( h UC-MSC) transplantation in treating ischemic stroke.Methods Seventy-five rats were randomized into a sham-operation group,a model group,a Chinese herbs group,a stem cell group and a combination group,with 15 rats in each group. The model of middle cerebral artery ischemiareperfusion was built in each group except for the sham-operation group. Since the first day after modeling,the Chinese herbs group and the combination group were given 11. 6 g/kg of SFD by gavage,while the other 3 groups were given 10 ml/kg of distilled water by gavage,once a day for 15 days. The stem cell group and the combination group were gently injected 1 × 106 h UC-MSCs through caudal vein on the first day after modeling. The neural function was scored 24 h before operation,and 2 h,the 4th day,8th day and 15 th day after operation. The apoptotic cells were marked by TUNEL staining on the 4th,8th and 15 th day after modeling respectively,and the serum expressions of tumor necrosis factor α( TNF-α),interleukin 1β( IL-1β),interleukin 6( IL-6) and interleukin 10( IL-10) were detected. Results Compared with those in the model group,the neural function scores lower,the apoptotic cell number reclined,the expressions of IL-1β and IL-6 dropped,and IL-10 expression increased in the Chinese herbs group on the 8th and 15 th day,as well as in the stem cell group and the combination group on the 4th,8th and 15 th day after modeling( P 〈0. 05 or P 〈0. 01); compared with that in the Chinese herbs group,the expression of TNF-αlowered in the combination group on the 4th and 8th day( P 〈0. 05); compared with those in the Chinese herbs group and the stem cell group,the expressions of IL-1β and IL-6 decreased on the 8th day,and the neural score lowered,while the apoptotic cell number decreased,and the expression of IL-10 raised on the 8th and 15 th day in the comb
