15902例门诊患者人乳头瘤病毒亚型感染情况分析

摘　　要：目的分析门诊患者人乳头瘤病毒(human papillomavirus,HPV)亚型感染情况,为宫颈疾病的防治提出依据。方法收集2013~2015年四川省妇幼保健院宫颈门诊15 902例患者HPV检测及病理诊断结果,统计分析门诊患者HPV感染情况、阳性患者宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)≥Ⅱ级患病情况、21种基因型的分布情况及其在≥CINⅡ患者中分布情况。结果 2013~2015年,检测基数逐年增加,HPV感染率逐年增高,且有递增趋势(P<0.05),但≥CINⅡ构成比逐年降低,且有递减的趋势(P<0.05);所有患者中,主要感染亚型为HPV 52、16、58、53、18;在≥CINⅡ患者中,主要感染亚型为16、58、52、33、18,与总体感染亚型分布有一定差异。结论 2013~2015年,我院宫颈门诊HPV感染率增高,≥CINⅡ构成比低,感染亚型及其致病性存在地区性。Objective To provide the basis for the prevention and treatment of cervical diseases by analyzing the infection of human papillomavirus（HPV） subtypes. Methods 15 902 cases of HPV detection and pathological diagnosis were collected from 2013 to 2015 in cervical clinic of Sichuan Provincial Hospital for Women and Children. The distribution of HPV infection in outpatients, the prevalence of cervical intraepithelial neoplasia （CIN） ≥ Ⅱ , the distribution of 21 genotypes and the distribution of them in ≥ CIN Ⅱ were analyzed statistically. Results From 2013 to 2015, HPV infection rate was significantly increased year by year（ P 〈 0. 05 ）, but the proportion of CIN Ⅱ decreased year by year （P 〈 0. 05 ） . Among all patients, 52, 16,58,53 and 18 were the main infection subtypes,while 16,58,52,33 and 18 were the main infection subtypes in CIN Ⅱ patients which have some differences compared with the distribution of the general infection subtypes. Conclusion From 2013 to 2015, HPV infection rate detected in our hospital increased, however, the proportion of CIN Ⅱ and above decreased. Infectious subtypes and their pathogenicity are regional.
