细节管理在社区糖尿病肾病血液透析患者中的效果观察

摘　　要：目的:探讨细节管理在社区糖尿病肾病血液透析患者中的应用效果。方法:选取2015年1月至2016年1月叠彩区、秀峰区及象山区在我院血液透析(HD)中心进行血液透析的糖尿病肾病患者128例,将患者随机分为观察组、对照组。对照组64例患者应用常规管理进行护理,观察组64例患者应用细节管理进行护理干预。比较两组患者的生活质量以及相关生化指标。结果:观察组患者管理后的LSR、LSIA、LSIB以及血红蛋白、白蛋白较对照组明显升高;观察组患者的肌酐较对照组明显降低,两组比较差异有统计学意义(P<0.05)。结论:细节管理能够有效改善血液透析糖尿病肾病患者血红蛋白、肌酐等相关指标,并提高患者的生活质量及满意度。Objective： To explore the effect of detail management in hemodialysis patients with diabetic nephropathy. Methods： From Jan. 2015 to Jan. 2016 in Diecai District,Xiufeng District and Xiangshan District in our hospital blood dialysis center,128 patients with diabetic nephropathy in hemodialysis patients were randomly divided into observation group and control group. 64 patients in the control group were treated with routine nursing,and 64 patients in the observation group were treated with detail management. The quality of life and related biochemical parameters of the two groups were compared.Results： Patients in the observation group of LSR,LSIA,LSIB and hemoglobin,albumin was significantly higher than the control group,the creatinine in the observation group was significantly lower than that of the control group, the difference was statistically significant（P〈0.05）. Conclusion： The detail management can effectively improve the hemoglobin,creatinine and other related indicators of diabetic nephropathy in hemodialysis patients,and improve the quality of life and satisfaction of patients.
