摘　　要：目的了解10余年来恩施州孕产期保健服务变化情况及存在的问题,为妇幼保健工作提供参考。方法通过常规报表和问卷调查收集资料,对恩施州1999-2014年的孕产期保健指标进行分析。结果 1999-2014年间,恩施州的产前检查率、住院分娩率分别由1999年的75.7%和27.2%升至2014年的98.3%和99.8%,剖宫产率从12.7%增至43.6%。产前检查的相关因素从其他孕产期保健指标向社会人口与资源配置因素和孕产期保健指标转变;住院分娩率的相关因素则从社会经济指标转变为卫生院妇幼保健人员配置和孕早期保健指标。结论恩施州孕产期保健指标有较大改善,但存在剖宫产率高等不足;提高基层孕产妇系统管理率和降低剖宫产率是下阶段的工作重点。Objective To understand the variation trend of maternal health care service and the existing problems in Enshi Tujia and Miao Autonomous Prefecture from 1999 to 2014,provide reference for maternal and child health care work.Methods The data of conventional reports and questionnaires about maternal health care indexes in Enshi Tujia and Miao Autonomous Prefecture from 1999 to 2014 were collected abd analyzed.Results From 1999 to 2014,the rates of prenatal examination and hospital delivery increased from 75.7% and27.2% in 1999 to 98.3% and 99.8% in 2014,respectively,the cesarean section rate increased from 12.7% in 1999 to 43.6% in 2014.The related factors of prenatal examination transformed from maternal health care indexes to social population and resource allocation factors and maternal health care indexes.The related factors of hospital delivery rate transformed from socio-economic factors to the indexes of allocation of maternal health care workers in health centers and antenatal care.Conclusion Great progress has been made in the field of antenatal care in Enshi Tujia and Miao Autonomous Prefecture,but there are some disadvantages,such as high cesarean section rate;the key emphasis in maternal and child health care work includes improving maternal system management rate in primary hospitals and reducing cesarean section rate.
