摘  要:目的探讨腔内心电图在瓣膜式经外周静脉置入中心静脉导管(PICC)头端定位中应用效果。方法选择2016年5~12月该院收治的拟行周期性静脉滴注化疗药物需置入瓣膜式PICC的恶性肿瘤患者107例,按照随机数字表法分为观察组(n=53)和对照组(n=54)。观察组和对照组中,分别有中3、4例因PICC置管过程中穿刺失败而被剔除本试验,最终纳入观察组和对照组各50例。观察组予以腔内心电图行瓣膜式PICC头端定位,对照组予以超声探查行PICC头端定位,置管后两组患者均行胸部X线摄片。比较两组导管头端到位准确率、导管头端异位率、置管所用时间及术中出血量。结果观察组导管头端到位准确率明显高于对照组,观察组导管头端异位率、置管所用时间、术中出血量明显低于对照组,差异均有统计学意义(P<0.05)。结论腔内心电图定位技术能够有效地减少瓣膜式PICC头端异位,值得临床推广应用。Objective To investigate the application effect of intravenous electrogram in the placement of the central ve-nous catheter tip through the Groshong peripherally inserted central catheter. Methods Select the patients from May 2016 to December 2016 who received periodic infusion chemotherapy drugs should be used with Groshong PICC were randomly divided into experimental group(n=53)and control group(n=53). In the observation group and the control group,3 cases and 4 cases were excluded because of the puncture failure in the process of PICC catheter insertion,and finally into the observation group and the control group,each group had 50 cases. The experimental group was given the PICC end position of the cardiac valve by intra-venous electrocardiogram,the control group was treated with ultrasonic probe to locate the catheter tip. The accuracy rate of catheter tip placement,the rate of ectopic end of catheter,the time of catheterization and the amount of intraoperative bleeding were recorded. Results The accuracy rate of catheter head in the experimental group was significantly higher than that in the control group( P〈0. 05). Conclusion The technique of intravenous electrogram can effectively reduce the ectopic end of Groshong PICC and improve the success rate of catheterization,which is worthy of clinical application.
