单类内隐联想测验评估自杀意念的有效性初探

摘　　要：目的探究单类内隐联想测验(SC-IAT)评估抑郁或/和焦虑状态青年患者自杀意念的有效性。方法选取169名抑郁和/或焦虑状态青年患者进行贝克自杀意念量表(BSI)和SC-IAT测试并评估是否有自杀计划,据此随机抽取被试进行分组,自杀意念组、自杀计划组和对照组各有28名。结果全体被试中,外显自杀意念、内隐自杀意念和自杀计划的报告率分别为40.24%、25.44%和16.57%。外显自杀意念者、内隐自杀意念者和混合自杀意念者中,自杀计划报告率分别为41.18%、41.86%和52.94%。外显、内隐测量结果有中等一致性(Kappa值=0.44,P<0.01)。单样本t检验发现,自杀计划组、自杀意念组D值均值大于0(P<0.01,P<0.05),对照组D值均值小于0(P<0.01)。方差分析表明组间D值差异有统计学意义(P<0.05)。Turkey事后检验显示,两两比较D值差异均有统计学意义(P<0.05)。结论 SC-IAT可以初步用于临床自杀意念的评估,是外显评估工具的重要补充。Objective To investigate the effects of single category implicit association test（ SC-IAT） to evaluate suicide ideation in young patients with depression and anxiety. Methods A total of 169 young patients with depression and anxiety were assessed with Beck Scale for Suicide Ideation（ BSI）,SC-IAT and divided into suicide ideation group,suicide plan group and control group and there were 28 patients in each group randomly. Results There were 40. 24% for the rate of explicit suicide ideation,25. 44% for the rate of implicit suicide ideation and 16. 57% for the rate of suicide plan in all subjects. The rates of suicide plan were 41. 18% for participants possessing explicit suicide ideation,41. 86% for participants possessing implicit suicide ideation and 52. 94% for participants possessing mixed suicide ideation. There was middle level for the consistency between two measurement tools（ Kappa = 0. 44,P〈0. 01）. The average values of D in suicide plan group and suicide ideation group were all significantly more than 0（ P〈0. 01,P〈0. 05） and the average value of D in control group was significantly less than 0（ P〈0. 01）. There were all significant differences for the average values of D among three groups（ P〈0. 05）. Conclusion It ＇s effective for SC-IAT to evaluate clinical suicide ideation, which should be imperative supplement for explicit measurement.
