摘  要:目的研究磷酸化表皮生长因子受体(P-EGFR)、磷酸化细胞外信号调节激酶(P-ERK)和细胞核转录因子κB(NF-κB)在中耳胆脂瘤上皮组织中的表达情况,探讨EGFR/ERK/NF-κB信号通路在中耳胆脂瘤发病机制中的作用。方法应用免疫组织化学SP法及Western blot(WB)免疫印迹法检测30例中耳胆脂瘤标本及15例正常外耳道皮肤组织中P-EGFR、P-ERK和NF-κB蛋白的表达。结果(1)P-EGFR蛋白阳性表达主要定位于细胞质和(或)细胞膜,在胆脂瘤组的阳性表达率为63%,明显高于正常外耳道皮肤组的20%(P<0.05);P-ERK蛋白阳性表达主要定位于细胞质和(或)细胞核,在胆脂瘤组的阳性表达率为70%,明显高于正常外耳道皮肤组的20%(P<0.05);NF-κB蛋白阳性表达主要定位于细胞核,在胆脂瘤组的阳性表达率为60%,明显高于正常耳道皮肤组的13.3%(P<0.05)。在30例中耳胆脂瘤组织中,P-EGFR、P-ERK和NF-κB两两之间呈正相关关系(P<0.05)。(2)Western blot免疫印迹法检测结果显示:中耳胆脂瘤组中P-EGFR、P-ERK和NF-κB的表达量明显高于正常外耳道皮肤组的表达量。结论 P-EGFR、P-ERK和NF-κB在中耳胆脂瘤上皮中表达增强,EGFR/ERK/NF-κB信号通路可能在中耳胆脂瘤组织的形成和发展过程中起重要作用。Objective To study the expression of P-EGFR,P-ERK and NF-κB in middle ear cholesteatoma epithelium,and explore the potential roles of EGFR/ERK/NF-κB signaling transduction pathway in middle ear cholesteatoma epithelial hyperplasia.Methods SP immunohistochemical assay and Western blot were used to detect the expression and significance of P-EGFR,P-ERK and NF-κB in 30 cases of middle ear cholesteatoma tissue samples and 15 cases of normal ear skin specimens.Results(1) Immunohistochemistry showed P-EGFR was expressed in the cytoplasm and/or membrane of epithelial cells.P-EGFR was positive in 19 of the 30 middle ear cholesteatoma specimens(63%),compared to 3 of the 15 specimens(20%) in the control group(P〈0.05).P-ERK expression in cholesteatoma was detected at a higher rate(21/30,70%) than normal external ear canal skin(3/15,20%)(P0.05).NF-κB immunoreactivity was detected in the nuclei of epithelial cells at a higher rate in cholesteatoma(18/30,60%) than the control group(2/15,13.3%)(P〈0.05).In middle ear cholesteatoma epithelium,there was a positive correlation between expression of P-EGFR,P-ERK and NF-κB(P〈0.05).(2) Western blot discovered that the expression of P-EGFR,P-ERK and NF-κB in cholesteatoma was significantly more than the amount of expression in skin.Conclusions The expression of P-EGFR,P-ERK and NF-κB in middle ear cholesteatoma was significantly higher than normal skin.Activation of EGFR/ERK/NF-κB signaling transduction pathway may be involved in the information and development of middle ear cholesteatoma.
