摘 要:目的:综合评价苦碟子注射液治疗脑梗死的有效性和安全性,为临床决策提供科学依据。方法:全面系统地检索中国知网、万方数据库、维普期刊数据库、中国生物医学文献服务系统、Cochrane library、Pub Med和Embase,收集苦碟子注射液治疗脑梗死的随机对照试验,检索时间为各数据库建库至2016年9月,采用Cochrane提供的偏倚风险评价工具对纳入研究进行质量评价,并用Rev Man 5.3软件进行统计分析。结果:共纳入22个研究,累计受试者2 353例。Meta分析结果显示,在西医常规治疗的基础上,联用苦碟子注射剂可明显提高临床疗效(RR=1.21,95%CI:1.16~1.26,P<0.000 1),苦碟子有效降低神经功能缺损程度(MD=-3.81,95%CI:-5.73^-1.88,P<0.000 01),可提高血浆黏度的好转程度(MD=-0.23,95%CI:-0.44^-0.02,P<0.000 01),提高红细胞压积的好转程度(MD=-0.02,95%CI:-0.05^-0.02,P<0.000 01)。结论:在西医常规治疗的基础上,应用苦碟子注射液辅助治疗脑梗死能提高临床总有效率,且在神经功能缺损、血浆黏度、红细胞压积等方面的疗效显著。Objective :To systematically evaluate the effectiveness and safety of Kudiezi injection (KDZ) as one adjuvant therapy for treating cerebral infarction (CI). Methods: Randomized controlled trials (RCTs) of CI treated by KDZ were searched in CNKI, Wanfang Database, VIP, Cochrane library, PubMed, SinoMed and Embase, which from the date of databases are built to September 2016. Cochrane Risk Assessment Tool was used to evaluate the methodological quality of the RCTs. The Review Manager 5.3 software were used to conduct the Meta-analysis of extracted data. Results : 22 RCTs including 2 353 patients were included finally. Meta-analysis results showed that the combined use of KDZ and western medicine(WM) in treatment of CI can obviously improve the total effective rate of cerebral infarction (RR = 1.21, 95% CI: 1.16-1.26, P 〈 0. 000 1 ), the degree of neurologic improvement (MD = - 3.81,95% CI: - 5.73- - 1.88, P 〈 0. 000 01 ), the plasma viscosity (MD = - 0.23, 95% CI: - 0. 44- - 0. 02, P 〈 0.000 01 ), the degree of hematocrit (MD = -0.02, 95% CI: -0.05- -0.02, P 〈0.000 01 ). Conclusion:Comparing with western medicine, the combine use of KDZ was more effectiveness in the treatment of cerebral infarction. Moreover, the efficacy of neural function defects, plas- ma viscosity and red blood cell deposition of which was remarkable.
