小针刀结合骨复生治疗早中期股骨头坏死疗效观察

摘　　要：目的观察小针刀结合骨复生治疗早中期股骨头坏死的临床疗效。方法将90例早中期股骨头坏死患者随机分为观察组和对照组各45例,观察组采用小针刀结合骨复生治疗,对照组采用局部封闭注射配合阿仑膦酸钠片口服治疗,2组均连续治疗6个月。观察2组治疗前后髋关节疼痛VAS评分及Harris评分,统计2组Harris评分结果疗效及临床疗效。结果治疗后2组VAS评分均明显低于治疗前(P均<0.05),Harris评分均明显高于治疗前(P均<0.05),且观察组VAS评分明显低于对照组(P<0.05),Harris评分中疼痛、关节畸形、关节活动评分及Harris总分均明显高于对照组(P均<0.05)。治疗后2组Harris评分优良率比较差异无统计学意义(P>0.05),但观察组临床总有效率明显高于对照组(P<0.05)。结论小针刀结合骨复生治疗早中期股骨头坏死临床疗效更好,且操作简单,易于被患者接受,值得临床推广应用。Objective It is to observe the clinical curative effect of small needle knife combined with Gufusheng on earlyto-middle stage osteonecrosis of femoral head. Methods 90 patients with early-to-middle stage osteonecrosis were randomly divided into observation group（n = 45） and control group（n = 45）. The observation group was treated with small needle knife combined with Gufusheng therapy. The control group was treated with local blocking and western medicine Alendronate tablets by oral administration. Both groups were treated for 6 months. The VAS score,Harris score of hip joint before and after treatment were observed and curative effects of Harris score and clinic were evaluated in both groups. Results After treatment,VAS scores were significantly lower while Harris scores were higher than those before treatment in both groups（P〈0. 05）,and VAS scores were lower,while Harris scores such as pain,joint deformation,joint mobility,and total Harris scores were higher in observation group than those in control group（P〈0. 05）. There was no significant difference in good rate of Harris scores between the two groups after treatment（P〉0. 05）,but the clinical total effective rate in observation group was higher than that in control group（P〈0. 05）. Conclusion Small needle knife release combined with Gufusheng has better curative effect on early-to-middle stage osteonecrosis of femoral head,its operation is simple and safe,so it is worthy to be popularized.
