PPH结合外痔切除术治疗重度混合痔的临床效果

摘　　要：目的比较传统痔切除术与PPH结合外痔切除术治疗重度混合痔的临床疗效。方法选取本院2012年12月至2015年12月收治的160例重度混合痔患者为研究对象,随机分为对照组(行传统痔切除术治疗,n=80)与观察组(行PPH结合外痔切除术,n=80),比较两组手术时间、术中出血量、住院时间、疼痛评分、临床疗效及术后并发症情况。结果观察组患者临床总有效率(91.25%)显著高于对照组(78.75%),差异具有统计学意义(P<0.05);观察组手术时间与对照组比较差异无统计学意义(P>0.05);观察组的住院时间、疼痛评分以及术后并发症发生率均低于对照组,差异均有统计学意义(均P<0.05)。结论与传统痔切除术相比,PPH结合外痔切除术治疗重度混合痔具有效果好、出血少、疼痛轻、术后并发症少的优点,患者住院时间短,值得临床推广。Objective To compare the clinical effects between PPH combined with external hemorrhoidectomy and traditional haemorrhoidectomy. Methods 160 patients with severe mixed hemorrhoid in our hospital between December 2012 and December 2015 were recruited and were randomly divided into control group（traditional haemorrhoidectomy treatment, n = 80） and the treatment group（PPH combined with external hemorrhoidectomy, n = 80）. We compared between the two groups regarding operation time, intraoperative blood loss, hospital stay, pain score, clinical curative effect, and postoperative complications. Results The clinical total effective rate in the treatment group（91.25%） was significantly higher than in the control group（78.75%）（P〈0.05）. The operating time in the treatment group was similar to that in the control group（P〉0.05）. the length of hospital stay, pain score and postoperative complication in the treatment group were shorter or lower than in the control group（P〈0.05）. Conclusion Compared with the traditional haemorrhoidectomy, PPH combined with external hemorrhoidectomy treatment has good effect for severe mixed hemorrhoids. It has less bleeding, fewer pain, less postoperative complications, and shortened hospitalization.
