IL-6和CRP在小儿复杂性阑尾炎行不同手术治疗的对比观察

摘　　要：目的对比分析IL-6、CRP等炎性应激指标在腹腔镜阑尾切除术(LA)与开腹阑尾切除术(OA)治疗小儿复杂性阑尾炎中的表现,以评价腹腔镜治疗小儿复杂性阑尾炎的效果。方法回顾分析手术治疗小儿复杂性阑尾炎200例临床资料,其中LA组120例,OA组80例。比较两组白细胞计数、超敏C反应蛋白、IL-6等炎性应激指标及术后肛门排气时间、术后恢复进食时间、术后疼痛等肠功能恢复情况和腹腔感染发生率、肠梗阻发生率、切口感染发生率、肠梗阻发生率等并发症,得出结论。结果 LA组与OA组相比,术前测量炎性指标,差异无统计学意义(P>0.05),LA组比OA组,术后白细胞计数、超敏C反应蛋白、IL-6低,术后恢复时间短,差异有统计学意义(P<0.05)。LA组比OA组,术后肠功能恢复较快,术后并发症较少。结论 LA组比OA组创伤小、恢复快、并发症少、炎症消退快等优点,是治疗小儿复杂性阑尾炎安全、有效的手术方式。Objective Comparative analysis of IL-6,CRP and other indicators of inflammatory stress in laparoscopic appendectomy（ LA） and open appendectomy（ OA） in the treatment of complex appendicitis in children,to evaluate the effect of laparoscopy in the treatment of complicated appendicitis in children. Methods The clinical data of 200 children with complicated appendicitis were retrospectively analyzed,including LA group（ n=） and OA group（ n = 80）. Comparison of two groups of white blood cell count,high sensitive C reactive protein and IL-6 inflammatory stress index and postoperative anal exhaust time,postoperative recovery time,postoperative pain,recovery of intestinal function and the abdominal infection rate,incidence of incision infection,incidence of intestinal obstruction and other complications,then draw a conclusion. Results Compared with the OA group,the preoperative measurement of inflammatory index in LA group was not statistically significant（ P 〉 0. 05）. Compared with group OA,the LA group had lower WBC count,high sensitivity C reactive protein,IL-6,and shorter postoperative recovery time,the difference was statistically significant（ P 〈 0. 05）. In group LA,the recovery of intestinal function was faster than that of OA group,and the postoperative complications were less. Conclusion Compared with OA group,the LA group has the advantages of less trauma,faster recovery,fewer complications,faster inflammation,and so on. It is a safe and effective surgical method for the treatment of complex appendicitis in children.
