摘  要:目的了解2015年武汉市发热呼吸道症候群病原谱构成。方法采用实时荧光定量PCR(Real-time PCR)方法从2015年1-12月,对采自武大中南医院和湖北省妇幼保健院共104份发热呼吸道症候群病例标本进行15种传染病病原体及分型的检测,分析其病原谱构成。结果共检测出11种传染病病原体阳性,38份阳性病例,总阳性率为36.54%(38/104),其中,病毒阳性29份,细菌阳性17份,细菌病毒共感染8份,分别占阳性病例的76.32%、44.74%和21.05%。男性感染阳性率32.75%(19/58),女性感染阳性率为41.3%(19/46),二者差异无统计学意义(χ~2=0.808,P=0.369)。5岁及以下婴幼儿是呼吸道病毒的主要感染人群。11种病原体中,排名前3位的为呼吸道合胞病毒(45.45%,5/11))、呼吸道腺病毒(36.36%,4/11))、肺炎链球菌(27.27%,3/11)。不同病原体感染季节性不同,有的病原体表现出严格的季节性。结论 2015年武汉市呼吸道症候群感染的病原体以病毒性病原体为主,尤其在低龄幼儿组,呼吸道合胞病毒、腺病毒和肺炎链球菌感染为其呼吸道症候群感染的主要病原体。Objective To investigate the pathogenic spectrum of respiratory syndrome in Wuhan City in 2015. Methods Real-time PCR assay was used to detect pathogens of 15 infectious diseases in 104 samples collected from Zhongnan Hospital of Wuhan University and Maternal and Child Health Care Hospital of Hubei Province from January to December, 2015. The composi- tion of the pathogenic spectrum was analyzed. Results Altogether 11 kinds of pathogens were detected in 38 cases, with the total positive rate of 36.54% ( 38/104 ). Among them, 29( 76.32% ) cases were tested positively for viruses and 17 ( 44.74% ) for bacte- ria and 8(21.05%) for coinfection. The positive rates of males and females were 32.75%(19/58) and 41.3%( 19/46 ) respective- ly, indicating no significant difference (χ2= 0.808, P= 0.369). Infants and young children aged five years old and below were the main inflicted group. The pathogens with the top three constitutions were respiratory syneytial virus (45.45%,5/11) , respiratory adenovirus (36.36%,4/11) and Streptococcus pneumoniae (27.27%,3/11). The infections with different pathogens showed dif- ferent seasonality, and some pathogens displayed strictly seasonal features. Conclusions Viral pathogens are the main patho- genic bacteria inducing respiratory tract infection syndrome in Wuhan City in 2015, especially in young children, Respiratory syn- eytial virus, human adenovirus and Streptococcus pneumoniae are the dominant pathogens.
