摘 要:目的 :分析卵巢癌的超声及MRI图像特点,探讨超声及MRI联合应用对卵巢癌的诊断价值。方法 :回顾性分析经手术病理证实的40例卵巢癌的影像学资料,术前均行超声及MRI检查,并与病理结果进行对比分析。结果:40例中,术前经超声诊断卵巢恶性肿瘤32例,诊断准确率为80.0%(32/40),其中实质性8例、混合性22例、囊性2例,误、漏诊8例。术前MRI诊断卵巢恶性肿瘤35例,诊断准确率为87.5%(35/40),MRI表现为单侧或双侧卵巢区的实性或囊实性肿块,肿块多呈等T1稍长T2信号,增强扫描后实性部分明显强化;其中12例伴大量腹腔积液,4例网膜转移;5例误、漏诊。超声与MRI联合应用诊断卵巢癌的准确率为95.0%,明显高于单一检查。结论:超声可对卵巢癌进行初步诊断,而MRI对卵巢癌定性诊断具有较高的准确率,两者联合应用,可有效提高卵巢癌的诊断准确率,尤其对早期卵巢癌的诊断有重要价值。Objective:To analyze the ultrasound (US) and magnetic resonance image (MRI) features of ovarian cancer,and to investigate the diagnostic value of combined ultrasound and MRI in ovarian cancer. Methods:Retrospective analyze imaging da-ta of clinical surgical or pathology confirmed 40 ovarian cancer cases ,who preoperative carried on the US and MRI examination at the same time,and compared with pathological results. Results:40 patients with ovarian cancer,preoperative diagnosd by US of 32 cases of ovarian malignant tumors,diagnostic accuracy was 80.0% (32/40),including 8 cases of solid,22 cases of mixed, and 2 cases of cystic,8 cases misdiagnosed. 35 cases of ovarian malignant tumors were diagnosed by MRI,diagnostic accuracy was 85.0% (34/40),magnetic resonance imaging (MRI) on the performance of unilateral or bilateral ovaries solid or cystic mass,mass formed iso- T1 and a bit long T2 signals,such as solid part obvious reinforcement after enhanced scanning. Accom-panied by a large number of peritoneal effusion,12 patients with omentum metastases in 4 patients. 5 cases of tumor confirmed by pathology of MRI. US with MR combined use of the accuracy in diagnosis of ovarian cancer was 95.0%,significantly higher than the separate checks. Conclusion:US preliminary diagnosis of ovarian cancer,while MR qualitative diagnosis of ovarian cancer with high accuracy,the joint application,can effectively improve the sensitivity and accuracy of the diagnosis of ovarian cancer,especially for early diagnosis of ovarian cancer has important value.
