和胃汤联合西药序贯治疗幽门螺杆菌相关性胃炎随机平行对照研究

摘　　要：[目的]观察和胃汤联合西药序贯治疗幽门螺杆菌相关性胃炎疗效。[方法]使用随机平行对照方法,将80例门诊患者按就诊顺序号抽签方法简单随机分为两组。对照组40例序贯疗法:前5d,奥美拉唑20mg/次+阿莫西林50mg/次,3次/d,口服;第6~10d,奥美拉唑20mg/次+阿莫西林50mg/次+克拉霉素1片/次+替硝唑10mg/次,2次/d,口服。治疗组40例和胃汤:谷芽、麦芽各15g,神曲、鸡内金、焦山楂、陈皮、莱菔子、佩兰各9g,枳壳、苏梗、桔梗、藿香各6g,甘草3g;乏力加党参、黄芪各9g;反酸加瓦楞子煅9g;胃痛加白芍9g,香附10g,水煎400mL,1剂/d,早晚分服;西药治疗同对照组。连续治疗10d为1疗程。观测临床症状、胃液pH、不良反应。连疗1疗程(10d),判定疗效。[结果]治疗组痊愈10例,显效10例,有效18例,无效2例,总有效率95.00%;对照组痊愈5例,显效8例,有效12例,无效15例,总有效率62.5%;治疗组疗效优于对照组(P<0.01)。胃液pH两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。不良反应发生率治疗组低于对照组(P<0.05)。[结论]和胃汤联合西药序贯治疗幽门螺杆菌相关性胃炎,疗效满意,无严重不良反应,值得推广。[Objective] To observe the therapeutic effect of Hewei decoction combined with western medicine in the treatment of helicobacter pylori associated gastritis. [Method] 80 cases of outpatients were randomly divided into two groups according to the number of patients in the treatment group. The control group of 40 cases： sequential therapy before 5d, omeprazole＋amoxicillin 20 mg/times 50 ragtimes, 3 times/d, oral; 6-10d times 50 mg/times 20 mg/omeprazole＋amoxicillin ＋ clarithromycin＋tinidazole 10 rag/1 tablet/time, 2 times/ d, oral. The treatment group of 40 cases Hewei decoction： Guya, Maiya every 15g, Shenqu, Jineijin, Xiaosha nzhai,Chenpi,Laifuzi,Lanpei every 9g, Zhiqiao, Sugeng, Jigeng, Huoxiang every 6g, Gancao 3g; hypodynamic plus Dangshen,Huangqi every 9g; sour regurgitation plus Walengzi 9g; Stomachache plus Baishao 9g, Xiangfu 10g, decoction 400mL, 1 doses of/d, sooner or later the service; westernmedicinewiththecontrolgroup. Continuous treatment of 10d for 1 courses. Observe clinical symptoms, gastric juice pH, adverse reactions. 1 courses of treatment （10d）, to determine the efficacy. [Results] the treatment group 10 cases were cured, 10 cases markedly effective, 18 cases effective, 2 cases ineffective, the total efficiency of 95%; control group 5 cases were cured, 8 cases markedly effective, 12 cases effective, 15 cases ineffective, the total efficiency of 62.5%; the treatment group than the control group （P〈0.01）. Gastric juice pH two groups were improved （P 〈0.01）, the treatment group was better than the control group （P〈0.01）. The incidence of adverse reactions in the treatment group was lower than that in the control group （P〈0.05）. [Conclusion] The treatment of Helicobacter pylori associated gastritis with Hewei decoction combined with western medicine is effective and has no serious adverse reactions.
