经验法与预注射试验法扫描启动时间在冠状动脉CTA的应用价值比较

摘　　要：目的研究于冠状动脉CTA中应用经验法和预注射试验法扫描启动时间的价值。方法资料选取本院2014年5月—2016年5月收治的疑似冠心病112例患者予以回顾性地分析,所有患者均采用冠状动脉CTA进行检查,按照临床所用不同方法分为2组,将应用预注射试验法50例患者作为对照组,将应用经验法62例患者作为观察组,且对扫描启动时间加以确定,对比两组血管强化度及不同强化度患者分布情况。结果两组阳性患者主动脉不同强化等级所占比例和阳性病变严重度分级对比均无明显差异(P>0.05);观察组BMI、肺动脉、升主动脉血管强化值(24.30±4.58)kg/m^2、(218.48±103.20)HU、(414.96±88.62)HU和对照组(24.68±2.71)kg/m^2、(236.15±79.60)HU、(402.10±81.35)HU对比均未显示高度差异(P>0.05)。结论冠状动脉CTA中应用经验法和预注射试验法扫描启动时间的价值高,有利于为疾病诊治提供重要参考,且经验法扫描启动时间可满足64排螺旋CT冠脉CTA的强化值,对对比剂使用剂量起到节省作用,且减轻碘负荷负面作用,有利于心功能疾病筛查。Objective To study the application value of experience method and preinjection test for scanning start time in coronary artery CTA. Methods A retrospective analysis of 112 patients suspected coronary heart disease in author＇s hospital from May 2014- May 2016 was taken in this study. All the patients were checked with coronary artery CTA and divided into 2 groups according different above method;pre-injection test was used in 50 patients who were divided as the control group,experience method was used in 62 patients as the observation group;the starting time was scanned;the vascular enhancement degree and distribution of different contrast enhancement were compared between both groups. Results In the patients with different aortic enhancement their acounted proportion and staging of positive lesion severity showed no significant difference （P〉 0.05） between both groups;in observation group BMI,pulmonary artery,ascending aortic vascular enhancement value （24.30±4.58） kg/m^2, （218.48±103.20） HU, （414.96±88.62） HU and in control group （24.68±2.71） kg/m^2, （236.15±79.60） HU, （402.10±81.35）HU both group＇s comparison all showed no higher difference （P〉0.05）. Conclusion For coronary artery in CTA experience method and preinjection test method to scan starting time have high value,can provide an important reference for the diagnosis and treatment of diseases,and experience mehtod for starting scan time can meet the reinforcement value of 64 slice spiral CT coronary artery CTA,and save the dosage of contrast medium,as well lessen the negative effects of iodine load,is conducive to heart function screening for the disease.
