摘 要:目的 分析中老年健康体检人群空腹血糖和糖化血红蛋白(HbA1c)检测结果,为中老年糖尿病的防治提供依据。方法 收集2015年1—12月来山东大学附属省立医院健康查体中心检查的2 570名中老年体检者的临床资料进行分析。空腹血糖采用己糖激酶方法检测,HbA1c采用高效液相色谱方法检测。针对空腹血糖和HbA1c在不同年龄、性别人群中的检测结果进行统计分析。结果 应用世界卫生组织空腹血糖诊断标准,糖尿病检出率为15.3%(392/2 570),男性检出率为18.0%(322/1 786),明显高于女性的8.9%(70/784)(P〈0.01);不同年龄组之间糖尿病检出率差异有统计学意义(P<0.01),60~69岁年龄段为21.9%(107/489),70岁及以上年龄段为18.0%(60/334),50~59岁为15.6%(143/919),40~49岁为9.9%(82/828)。应用世界卫生组织HbA1c诊断标准,糖尿病检出率为17.1%(439/2 570),男性检出率为19.2%(343/1 786),明显高于女性的12.2%(96/784)(P〈0.01);不同年龄组之间的糖尿病检出率差异有统计学意义(P〈0.01),检出率最高的年龄段为≥70岁,为25.7%(86/334),60~69岁年龄段为25.6%(125/489),50~59岁年龄段为15.9%(146/919),40~49岁检出率为9.9%(82/828)。结论 在中老年健康体检人群中男性糖尿病检出率均明显高于女性,且糖尿病检出率随着年龄增长呈升高趋势。Objective To analyze fasting plasma glucose(FPG) and glycosylated hemoglobin(HbA1c) levels in middle and old age people. Methods A total of 2 570 middle and old age people who had physical examination from January to December 2015 in Shandong Provincial Hospital Affiliated to Shandong University were retrospectively analyzed. FPG was tested by hexokinase method and HbA1c was tested by high performance liquid chromatography method. Detection results of FPG and HbA1c were analyzed. Results According to results of FPG of World Health Organization criteria, the prevalence rate of diabetes in 2 570 people was 15.3%(392/2 570); it was 18.0%(322/1 786) in male and 8.9%(70/784) in female(P〈0.01). There was a significant difference of diabetes rate among different age groups(P〈0.01); the diabetes rate was 21.9%(107/489) in 60-69 years old, 18.0%(60/334) in 70 years old and over, 15.6%(143/919) in 50-59 years old and 9.9%(82/828) in 40-49 years old. Using HbA1c as the diagnostic criteria, the diabetes rate was 17.1%(439/2 570); it was 19.2%(343/1 786) in male and 12.2%(96/784) in female(P〈0.01). The difference of diabetes rate among age groups was significant(P〈0.01); it was 25.7%(86/334) in ≥70 years old group, 25.6%(125/489) in 60-69 years old group, 15.9%(146/919) in 50-59 years old group and 9.9%(82/828) in 40-49 years old group. Conclusion The detection rate of diabetes in middle and old age male people is significantly higher than that in female and it increases with age.
