老年肥胖患者血浆视黄醇结合蛋白4水平与血压及胰岛素抵抗相关性研究

摘　　要：目的测定老年肥胖患者及正常者血浆视黄醇结合蛋白4水平,分析其与收缩压、舒张压、胰岛素抵抗、血糖、胰岛素、血脂、腰臀比、腰围、年龄的相关性。方法以老年肥胖患者43例为病例组,老年健康人群41例为对照组。测量血压、腰围、身高、臀围、体重。抽取空腹静脉血,测定血浆视黄醇结合蛋白4、胰岛素、血糖、血脂(甘油三酯、高密度脂蛋白胆固醇、总胆固醇、低密度脂蛋白胆固醇)水平,计算体重指数、腰臀比、胰岛素抵抗指数、体内脂肪百分比。结果老年肥胖患者血浆视黄醇结合蛋白4水平显著高于对照组(P<0.01);老年肥胖组患者血浆视黄醇结合蛋白4水平与年龄、收缩压、舒张压、体重指数、胰岛素、胰岛素抵抗指数、HDL-C呈正相关;对照组血浆视黄醇结合蛋白4水平与臀围、空腹血糖呈正相关,与其他指标无相关性。结论老年肥胖组患者血浆视黄醇结合蛋白4水平升高并与年龄、收缩压、舒张压、体重指数、胰岛素、胰岛素抵抗指数、HDL-C之间存在一定的相关性。Objective To determine the level of plasma retinol binding protein 4 in elderly obese patients and normal subjects, and analyze its relationship with systolic blood pressure, diastolic blood pressure, insulin resistance, blood glu- cose, insulin, blood lipid, waist to hip ratio, waist circumference and age. Methods The 43 obese elderly patients were selected as the case group, and 41 healthy elderly subjects were selected as the control group. The blood pressure, waist circumference, height, hip circumference, and body weight were measured. The fasting venous blood was collect- ed. The levels of plasma retinol binding protein 4, insulin, blood glucose, blood lipid （triglyceride, high density lipopro- tein cholesterol, total cholesterol, low density lipoprotein cholesterol） were determined, and body mass index, waist to hip ratio, insulin resistance index, and body fat percentage were calculated. Results The levels of plasma retinol bind- ing protein 4 in elderly obese patients were significantly higher than those in control group （P〈0.01）; The levels of plas- ma retinol binding protein 4 in elderly obese patients were positively correlated with age, systolic blood pressure, dias- tolic blood pressure, body mass index, insulin, insulin resistance index and HDL-C; there were positive correlations between plasma retinol binding protein 4 level and hip circumference and fasting blood glucose in the control group, but no correlation with other indices. Conclusion The levels of plasma retinol binding protein 4 in elderly obese pa- tients are correlated with age, systolic blood pressure, diastolic blood pressure, body mass index, insulin, insulin resis- tance index and HDL-C.
