防旋股骨近端髓内钉内固定治疗老年人股骨转子间骨折的临床观察

摘　　要：目的探讨防旋股骨近端髓内钉内固定(PFNA)治疗老年人股骨转子间骨折的临床疗效。方法选择2013年1月~2016年12月我院收治的老年股骨转子间骨折患者40例作为研究对象,根据手术方法不同分成观察组和对照组,每组20例,对照组给予动力髋螺钉(DHS)治疗,观察组给予防旋股骨近端髓内钉内固定治疗,比较两组手术情况、临床疗效、并发症发生率。结果观察组患者手术时间、手术切口长度短于对照组,术中出血量少于对照组,患者优良率高于对照组,并发症发生率低于对照组,差异有统计学意义(P<0.05)。两组患者骨折愈合时间比较,差异无统计学意义(P>0.05)。结论防旋股骨近端髓内钉内固定治疗老年人股骨转子间骨折的临床疗效显著,且手术时间短、术中出血量少,值得临床推广和应用。Objective To explore the clinical effect of proximal femoral nail antirotation （PFNA） in the treatment of in- tertrochanteric fractures in the elderly. Methods 40 with femoral intertrochanteric fractures in our hospital from January 2013 to December 2016 were selected and randomly divided into the observation group and the control group,with 20 cases in each group.The control group was given dynamic hip screw,while the observation group was given anti-rotation proximal intramedullary nail fixation treatment.The situation of operation,clinical efficacy and the incidence of complication were compared between two groups.Results The operation time and the length of operative incision in the observation group were shorter than tnose in the control group,the peroperative bleeding was less than that in the con- trol group,the incidence of complication was lower than that in the control group,and the differences were statistically significant （P〈0.05）.There were no significant differences on the fracture healing time between two groups （P〉0.05）. Conclusion The curative effect of anti-rotation femoral intramedullary nail fixation in the treatment of intertrochanteric fractures of the elderly is significantly better than that of the dynamic hip screw, and the operation time is short and the intraoperative blood loss is small, which can promote the rehabilitation of the patients. And application.
