风湿免疫病血液系统损害的主要临床表现及其治疗

摘　　要：目的了解临床上风湿免疫病患者血液系统受损的表现,分析其治疗的有效手段。方法随机选取2015年7月至2016年7月我院收治的风湿免疫病伴有不同程度血液系统损害的60例患者的病例资料进行回顾性分析,分析主要临床表现;所有患者均采用糖皮质激素治疗,统计其治疗效果。结果在103例风湿免疫病患者中,发生不同程度血液系统损害的有60例,占58.25%。患者的主要临床表现是贫血32例(53.33%)、白细胞数减少19例(31.67%)和血小板数减少9例(15.00%)。治疗后,贫血患者症状有所减轻的有23例,血小板减少患者症状减轻的有6例,均未发生感染等其他并发症,白细胞减少患者症状有所减轻的有11例,出现感染2例,死亡1例。结论风湿免疫病的血液系统损害主要是贫血、血小板减少和白细胞减少,其治疗基础都是使用糖皮质激素。Objective To explore the clinical features of blood system damage in patients with rheumatic diseases, and to analyze the effective means of treatment. Methods The clinical data of 60 cases of patients with different degrees of blood system damage and rheumatic diseases treated in our hospital from July 2015 to July 2016 were randomly selected and retrospectively analyzed, and its major clinical manifestations were analyzed. All the patients were treated with gluco- corticoid, and the results of treatment were recorded. Results Of 103 cases of patients with rheumatic diseases, 60 cases occurred varying degrees of blood system damage, accounting for 58.25%. The main clinical manifestations of the patients were anemia in 32 cases （53.33%）, white blood cell reduction in 19 cases （31.67%） and thrombocytopenia in 9 cases （15.00%）. After treatment, there were 23 cases of anemia symptoms relieved, 6 cases of white blood cell reduction relieved and they had no other complications such as infection; 11 cases of thrombocytopenia relieved, with 2 cases of infection and 1 case of death. Conclusion The main damage of the blood system in rheumatic diseases are anemia, thrombocytopenia, and white blood cell reduction, and its treatment is based on the use of glucocorticoids.
