多种护理方式对乳腺癌术后淋巴水肿的干预效果比较

摘　　要：目的上肢淋巴水肿是乳腺癌切除术后的常见并发症,本研究对多种不同护理方式在改善乳腺癌术后患者淋巴水肿的短期和长期效果进行了比较。方法选取我院2011年1月至2015年12月接受乳腺癌切除术治疗的102例患者,分入干预组(78例)和对照组(24例),其中干预组患者分别接受患肢功能锻炼26例、弹力袖带26例、按摩淋巴液导流26例。对术后1、3、6、12个月的淋巴水肿程度的评估参数进行比较。结果与对照组相比,接受不同干预的受试者在各个时间点处的水肿程度均有一定程度的改善,且78例干预术后护理受试者中,无一例严重淋巴水肿发生。干预组的生活质量也较对照组有显著改善,其中弹力袖带组的生活改善程度最为显著。结论预防性干预能显著降低乳腺癌术后淋巴水肿的发生率和严重程度。其中患者对于弹力袖带的顺应性最好。Objective Lymphatic edema of upper limb is a common complication after resection of breast cancer. In this article, we compared the efficiency of different nursing methods for postoperative lymphedema. Methods One hundred and two patients receiving breast cancer resection during last 5 years were recruited and al- located randomly into intervention group （n= 78） and control group （n=24）. Patients in intervention groups were treated with different caring methods, including additional exercise for the affected side （n= 26）, elastic cuff （n= 26）, and massage for lymph diversion （n= 26）. Edema condition at month 1, 3, 6 and 12 after resection were compared. Results Compared with control group, different nursing cares provided significant improvement at all monitored time points. Among the 78 subjects in intervention group, none of severe lymphatic edema was ob- served. The living quality of intervention group was also significantly improved compared with control group, among which elastic cuff provided most satisfactory effect. Conclusion Preventive intervention can significantly re- duce the incidence and severity of postoperative lymphatic edema in breast cancer patients, and the elastic cuff compliance contributed most significantly to daily living.
