中药汤剂联合腹腔镜下胆囊切除术治疗老年急性胆囊炎临床观察

摘　　要：目的观察中药汤剂联合腹腔镜下胆囊切除术治疗老年急性胆囊炎(AC)的临床疗效。方法将76例老年AC患者随机分为2组,治疗组38例予中药汤剂联合腹腔镜下胆囊切除术治疗,对照组38例予单纯腹腔镜下胆囊切除术治疗,中药汤剂连续服用2周,观察2组住院时间、术后肛门排气时间及下床活动时间,术后并发症发生情况,比较2组疗效。结果治疗组住院时间、术后肛门排气时间及下床活动时间均较对照组提前(P<0.05);治疗组总有效率97.3﹪,对照组总有效率89.5﹪,2组总有效率比较差异有统计学意义(P<0.05),治疗组疗效优于对照组;治疗组术后并发症发生率7.9%,对照组术后并发症发生率26.3%,治疗组术后并发症发生率低于对照组(P<0.05)。结论中药汤剂联合腹腔镜下胆囊切除术治疗老年AC,可促进患者恢复,缩短住院时间,降低围手术期并发症,提高手术整体疗效。Objective To observe the clinical effects of Chinese herb decoction combined with laparoscopic cholecystectomy on the treatment acute cholecystitis（AC） of elderly patients.Method 76 elderly patients with AC were randomly divided into two groups.The treatment group（n = 38） was treated by Chinese herb decoction combined with laparoscopic cholecystectomy,and the decoction was taken continuously for 2 weeks.The control group（n= 38） was treated by laparoscopic cholecystectomy.The hospital stays,anus exhausting time after operation,out of bed activity time and the incidence of postoperative complications were observed in two groups,and the curative effects were compared.Results The hospital stays,anus exhausting time after operation and out of bed activity time in treatment group were shorter than that in control group（P〈0.05）.The total effective rate in treatment group（97.3%） was superior to that in control group（89.5%,P〈0.05）,and the clinical effects in treatment group was superior to that in control group.The incidence of postoperative complicationsin treatment group（7.9%） was lower than that in control group（26.3%,P〈0.05）.Conclusion Chinese herb decoction combined with laparoscopic cholecystectomy on AC can promote the recovery of patients,shorten the hospital stay,reduce perioperative complications,and improve the overall effect of surgery.
