摘　　要：目的:本研究旨在通过观察心力衰竭患者血浆NT-proBNP水平动态变化,探讨盐酸曲美他嗪片在心衰治疗中的作用。方法:入选的48名心衰患者均应用抗心衰标准治疗(包括口服利尿剂、β受体阻滞剂、强心药、ACEI、醛固酮受体拮抗剂、血管扩张剂),治疗组在抗心衰标准治疗的基础上加用盐酸曲美他嗪片20 mg,3次/日口服12周。观察治疗前后两组患者血清NT-proBNP变化水平、6 min步行距离增减及患者心功能改善情况。结果:随着心功能分级增加,NT-proBNP水平呈显著上升(P<0.01);治疗12周后2组患者的血浆NT-proBNP水平均下降(P<0.01),6 min步行距离明显增长(P<0.01),且治疗组明显优于对照组(P<0.05)。结论:NT-proBNP可作为评估心衰患者心功能改善的一项敏感指标;在常规治疗的基础上加曲美他嗪可进一步改善患者心功能,增加其疗效。Objective We investigated the plasma levels change of NT-proBNP to approach the therapeutic effect of Trimetazidine in heart failure patients.Methods 48 patients with heart failure were randomly divided into 2 groups,all of them were treated by standard treatment(include oral diuretics、β-blockers、cardinatia、ACEI、Aldosterone receptor antagonist、vasodilating agent),24 patients of the treatment group were treated by Trimetazidine in the conventional treatment.The time lasted for 12 weeks,clinical efficacy were observed before and after treatment,including N-terminal pro-B-type natriuretic peptide(NT-proBNP),6 minute walk distance,assessing the improvement of heart function.Results With the decrease of heart function,NT-proBNP is gradually elevated(P<0.01);after 12 weeks later the plasma NT-proBNP in two group patients were descendent(P<0.01),6 minute walk distance were ascending(P<0.01),but the efficacy of treatment group was superiored than control group(P<0.05).Conclusion NT-proBNP could be a sensitive index to evaluate the improvement of heart function in heart failure;and the heart function was more improved after adding Trimetazidine to the conventional treatment.
