椎间盘镜下等离子刀髓核消融与小切口开放手术治疗钙化型腰椎间盘突出症的比较

摘　　要：目的:观察比较后路显微内窥镜腰椎间盘切除术(MED)结合等离子刀髓核消融与小切口开放手术治疗钙化型腰椎间盘突出症的临床疗效。方法:随机选择2005年6月～2010年6月手术治疗的钙化型腰椎间盘突出症患者56例,椎间盘镜下等离子刀髓核消融手术28例(A组),后路小切口开放手术28例(B组),观察二组手术疗效。结果:按Nakai治愈标准评估手术疗效,二组优良率比较有统计学差异(P<0.05)。椎间盘镜下等离子刀髓核消融组(A组)较小切口开放手术组(B组)术中出血量小,手术时间短,住院天数少,术后康复快。结论:椎间盘镜下等离子刀髓核消融治疗钙化型腰椎间盘突出症较开放性手术,具有创伤小、出血少、康复快和疗效好等特点。Objective To observe and compare the clinical effect of decompression by radiofrequency coblation nucleoplasty on Microendoscopic discectomy(MED) versus posterior microtrauma approach open operation in the treatment of the calcified lubar disc herniation.Methods 56 patients from June 2005 to June 2010 with the calcified lubar disc herniation were randomized into two groups.28 patients in group A underwent radiofrequency coblation nucleoplasty on MED while 28 patients in group B received open operation.Results The therapeutic effect was evaluated by Nakai standard.There was significant difference between the two groups(P<0.05).The volume of bleeding,the operation time and the days of hospitalization of the group A were significantly lower than the group B.Conclusion The radiofrequency coblation nucleoplasty on Microendoscopic discectomy to treat the patients with the calcified lubar disc herniation is simple and efficient and is of less trauma,less bleeding,rapid recovery.
