鼻内镜下鼻中隔矫正联合下鼻甲黏膜下部分切除术治疗伴鼻中隔偏曲的变应性鼻炎

摘　　要：目的探讨鼻内镜下鼻中隔黏膜下切除术联合下鼻甲黏膜下海绵体部分切除术对于伴鼻中隔偏曲的变应性鼻炎的治疗效果。方法研究对象选自我院2013年1月~2015年8月收治的48例伴鼻中隔偏曲的变应性鼻炎患者,采取随机数字表将其分成两组,每组24例。观察组行鼻内镜下鼻中隔黏膜下切除术联合下鼻甲黏膜下海绵体部分切除术,对照组行常规鼻内镜下鼻中隔黏膜下切除术。对所有患者进行为期1年的随访,比较两组术后1个月、6个月以及1年的手术疗效。结果观察组术后1个月总有效率为95.83%,对照组为91.67%,两组术后1个月总有效率比较无明显差异(P>0.05),观察组术后6个月、1年总有效率分别为95.83%、83.33%,与对照组的70.83%、50.00%相比均有明显上升(P<0.05);两组术后1个月、6个月、1年临床症状、临床体征评分均显著低于术前(P<0.05),观察组术后6个月、1年临床症状、临床体征评分明显低于对照组(P<0.05);观察组术后1个月、6个月NPT阴性率分别为91.67%、87.50%,对照组分别为83.33%、70.83%,两组同时点对比无明显差异(P>0.05),观察组术后1年NPT阴性率为79.17%,与对照组的45.83%相比有明显上升(P<0.05);两组术后1个月、6个月、1年各项鼻部症状VAS评分相对于术前均有明显下降(P<0.01),观察组术后6个月鼻塞VAS评分、术后1年鼻塞、鼻痒、流涕、喷嚏VAS评分均显著低于对照组同时点(P<0.01)。结论鼻内镜下鼻中隔黏膜下切除术联合下鼻甲黏膜下海绵体部分切除术治疗伴鼻中隔偏曲的变应性鼻炎与单纯鼻内镜下鼻中隔黏膜下切除术相比近期疗效相当,但远期疗效更为显著。Objective To explore the clinical effect of the endoscopic nasal septum and inferior turbinate cavernous partial resection in the treatment of variability rhinitis with nasal septum deviation. Methods 48 cases of variability rhinitis with nasal septum deviation patients treated in our hospital from January 2013 to August 2015 were selected and divided into two groups with the method of random number table, each group with 24 cases. The observation group underwent endoscopic submucosal resection of nasal septum and inferior turbinate cavernous resection, while the control group took conventional endoscopic submucosal resection of nasal septum. All patients were followed up for 1 year, and to compare the operation curative effect of the two groups after 1 month, 6 months and 1 year of the surgery. Results The total effective rate of the observation group was 95.83% after 1 month of the surgery, and the control group was 91.67%. There was no significant difference in total effective rate between the two groups after 1 month （ P〉0.05）. The total effective rate of the observation group was respectively 95.83%and 83.33%after 6 months and 1 year of the surgery, which increased significantly compared with 70.83% and 50.00% of the control group （P〈0.05）. The clinical symptoms and clinical signs scores of two groups at the 1st, 6th month and the 1st year after operation were significantly lower than those before operation （P〈0.05）. The clinical symptoms and clinical signs scores in observation group at the 6th month and the 1st year after operation were significantly lower than those of the control group （P〈0.05）. The negative rate of NPT at the 1st and 6th month after the operation in the observation group was respectively 91.67% and 87.50%, while that in the control group was 83.33% and 70.83% respectively.
