影响2型糖尿病患者股浅动脉闭塞腔内治疗远期通畅率的相关因素分析

摘　　要：目的:探讨2型糖尿病患者股浅动脉闭塞腔内治疗的远期通畅率及其相关影响因素。方法:选取本院2014年1月-2016年1月收治的2型糖尿病股浅动脉闭塞行腔内治疗的患者117例,另选取同时间段单纯股浅动脉闭塞行腔内治疗的患者117例作为对照组,先对可能影响患者腔内治疗术后远期血管通畅率的因素进行单因素分析,挑选出差异有统计学意义的因素,再行多因素logistic分析。结果:单因素分析中,可能影响腔内治疗术后远期血管通畅率的因素为空腹血糖、年龄、甘油三酯、高密度脂蛋白、低密度脂蛋白、ABI初始值以及血清Vaspin水平(P<0.05)。经过多因素的logistic分析,影响远期血管通畅率的独立危险因素为年龄、空腹血糖、低密度脂蛋白、血清Vaspin水平[P=0.021,95%CI(0.891,1.245);P=0.016,95%CI(0.991,1.345);P=0.014,95%CI(0.791,1.235);P=0.019,95%CI(0.761,0.945)]。结论:股浅动脉闭塞症合并2型糖尿病患者腔内治疗后远期血管通畅率与年龄、空腹血糖、低密度脂蛋白、血清Vaspin水平密切相关。Objective： To analyze the relative factors of long-term vascular pateney after intracavitarytherapy in patients with type 2 diabetes mellitus complicated with superficial femoral arterial occlusive disease. Method： 117 patients with type 2 diabetes mellitus complicated with superficial femoral arterial occlusive disease who received intracavitary therapy from January 2014 to January 2016 in our hospital were selected as the observation group, a total of 117 patients who underwent intracavitary treatment of superficial femoral artery occlusion at the same time were selected as the control group, the factors of vascular patency were analyzed by univariate analysis, and the factors with statistical differences were selected to preform multivariate logistic analysis.Result： In the univariate analysis, factors that may affect the long-term patency of endovascular treatment for postoperative fasting blood glucose, age, triglyceride, high density lipoprotein, low density lipoprotein, the initial value of ABI and the level of serum Vaspin （P〈0.05） .After multivariate logistic analysis, the independent risk factors for the long-term patency rate of age, fasting blood glucose, low density lipoprotein, the level of serum Vaspin[P=-0.021, 95%CI （0.891, 1.245 ） ; P=0.016, 95%CI （0.991, 1.345 ） ; P=0.014, 95%CI （0.791, 1.235 ） ; P=0.019, 95%CI （0.761, 0.945 ） ].Conclusion： Long-term vascular patency rate in patient with type 2 superficial diabetes mellitus complicated with superficial femoral arterial occlusive disease who underwent intracavitary therapy is closely related to age, fasting blood glucose, low-density lipoprotein and the level of serum Vaspin.
