会阴收缩力计辅助缩肛运动对骶骨固定术后尿动力学的研究

摘　　要：目的:探讨会阴收缩力计辅助缩肛运动对骶骨固定术后压力性尿失禁(SUI)预防和治疗作用及尿动力学改变情况。方法:选择2014-2016年因中重度盆腔器官脱垂施行腹腔镜阴道骶骨固定术的患者为研究对象,术后68例患者作为预防SUI的尿动力学研究对象,54例患者作为治疗SUI的尿动力学研究对象,两部分研究对象均采用随机数字表法分为对照组和试验组,预防研究部分每组34例,治疗研究部分每组27例。对照组采用常规的缩肛运动,试验组则采用会阴收缩力计辅助缩肛运动,训练时间均为1年,并采取尿动力检查指标进行评估。结果:SUI的预防研究中,试验组SUI发生率为5.9%,低于对照组的26.5%(P<0.05);SUI的治疗研究中,试验组治愈率为63.0%,高于对照组的22.2%(P<0.05)。两部分研究中,试验组VLPP、PQmax、PMUC、BC均高于对照组,差异均有统计学意义(P<0.05)。结论:会阴收缩力计辅助缩肛运动可改善和促进骶骨固定术后尿动力学指标,对SUI有预防和治疗作用。Objective： To study the perinea] contraction force assist shrinking anus movement after sacral fixation for prevention and treatment of stress urinary incontinence （ SUI ） and urinary dynamic change situation.Method： From 2014 to 2016, patients adopted laparoscopic vaginal sacral fixation because moderate-severe pelvic organ prolapse were selected as research object, after the fixation, 68 patients were studied urine dynamics s for prevented SUI, 54 patients were studieds urine dynamics for treated SUI, two parts study both divided patients into control group and experimental group according to the random number table method, prevention parts 34 patients in each group, treatment parts 27 patients in each group, the control group was adopted routine shrinking anus movement, the experimental group used perineal contraction force to assist shrinking anus movement, training time both was 1 year and evaluated by urine dynamics test indicators.Result： In prevention study, SUI incidence of experimental group was 5.9%, less than control group＇s 26.5% （ P〈0.05 ） ; in the treatment study, the cure rate of experimental group was 63.0%, higher than control group＇s 22.2% （P〈0.05 ） . the VLPP, PQmax, PMUC, BC of experimental group in both two parts of study were higher than those of the control group, the differences were statistically significant （P〈0.05） .Conclusion： Perineal contraction force can assist shrinking anus movement, improve and promote postoperative urinary dynamics index after sacrum fixation, prevent and treat SUI.
