头部电针透穴刺法治疗脑卒中后抑郁症的临床研究

摘　　要：目的:观察头部电针透穴刺法治疗脑卒中后抑郁症(PSD)的临床疗效并探讨其作用机理。方法:150例PSD患者,随机分为观察组(头部电针透穴组)50例、对照1组(西药组)50例和对照2组(普通针刺)50例,治疗30 d。治疗前后分别对3组患者进行HAMD、SDS量表评分和血浆5-HIAA含量测定,观察脑电图变化。结果:观察组有效率为86.0%,明显优于普通针刺组的58.0%和西药对照组的64.0%,经统计学处理,差异均有显著意义(P<0.05)。3组治疗后HAMD、SDS量表评分均明显降低,血浆5-HIAA含量明显升高,脑电图异常率明显降低,与治疗前比较,差异均具有统计学意义(P均<0.05);观察组效果更显著,与两对照组相比差异均有统计学意义(P<0.05)。结论:头部电针透穴刺法能有效治疗PSD,明显提高PSD病人血浆5-HIAA含量,改善PSD患者的脑电图异常。Objective：To observe the clinical curative effect and the mechanism of scalp electroacupuncture （EA） point-through -point therapy in the treatment of post stroke depression （PSD）. Methods： 150 cases of PSD patients were randomly divided into observation group （ scalp electro - acupuncture point - through - point group）, the control group I （Western medicine group）, the control group II（ ordinary acupuncture） with each 50 eases,they were treated for 30 days. Before and after treatment, HAMD, SDS scale score of the three groups patients were recorded, plasma 5 - HIAA level was determined and changes of EEG were observed. Results： The effective rate of the observation group was 86. 0%, which was significantly better than that of the ordinary acupuncture group （58.0%） and the Western medicine control group （ 64. 0% ）. The differences were statisti- cally significant （P 〈 0. 05 ）. HAMD and SDS scores of the three groups were significantly decreased after treat- ment, plasma 5 - HIAA content increased obviously, the abnormal rate of EEG was significantly reduced; com- pared with before treatment, the differences were statistically significant （ P 〈 0.05 ） ; the effects of the observa- tion group were the most significant, the differences were significant to compare with the two control groups re- spectively （ P 〈 0. 05 ）. Conclusion： Electro - acupuncture scalp point - through - point therapy can effectively treat PSD, raise plasma 5 - HIAA content and improve the abnormal EEG.
