针灸推拿联合颈前路减压植骨内固定治疗脊髓型颈椎病临床研究

摘　　要：目的评价针灸推拿联合颈前路减压植骨内固定治疗脊髓型颈椎病的疗效,并分析其作用机理,为临床防治提供指导。方法选取本院2013年10月~2014年10月收治的62例脊髓型颈椎病患者,按随机数表法分成治疗组和对照组,每组31例。对照组接受颈前路减压植骨内固定治疗,治疗组接受针灸推拿联合颈前路减压植骨内固定治疗。观察治疗前、后2组JOA评分、中医症候积分、VAS评分、生存质量评分变化情况,并对预后情况进行评价。结果治疗后2组JOA评分、生存质量评分较治疗前均明显上升,而中医症候积分、VAS评分均明显降低,治疗组改善效果优于对照着（P〈0.05）;治疗组预后优于对照组（P〈0.05）。结论采用针灸推拿联合颈前路减压植骨内固定治疗脊髓型颈椎病可显著提高临床疗效及预后,且安全性较高。Objective To evaluate the effect of acupuncture and massage combined with anterior cervical decompression and bone grafting for the treatment of cervical spondylotic myelopathy,and to analyze the mechanism of action and provide guidance for clinical prevention and treatment.Methods A total of 62 patients with cervical spondylotic myelopathy were enrolled in our hospital from October 2013 to October 2014. The patients were divided into treatment group and control group according to random number table. The control group was treated with anterior decompression and bone grafting,and the treatment group received acupuncture and massage combined with anterior cervical decompression and bone grafting and internal fixation. The changes of JOA score,TCM syndrome score,VAS score and quality of life score before and after treatment were observed and the prognosis was evaluated. Results After treatment,the JOA score,quality of life score of the two groups were significantly increased than those before treatment,while the TCM symptom scores,VAS scores were significantly decreased. The improvement effect of the treatment group was better than that of the control group（ P〈 0. 05）; The prognosis of the treatment group is better than that of the control group（ P 〈0. 05）. Conclusion The treatment of cervical spondylotic myelopathy with acupuncture and massage combined with anterior decompression and bone grafting and internal fixation can significantly improve the clinical efficacy and prognosis,and the safety is higher.
