摘 要:目的分析辅助生殖技术对出生新生儿性别比例的影响。方法追踪随访2012年起在广州医科大学附属第三医院生殖医学中心行辅助生殖治疗,成功妊娠并已分娩的患者资料,共4 062个周期,先分成新鲜胚胎移植组和解冻胚胎移植组,比较新生儿性别比例;然后把新鲜胚胎移植组按助孕方式分成体外受精(IVF)组和卵胞浆内单精子注射(ICSI)组,比较新生儿性别比例。根据移植胚胎在体外培养时间的长短,将所有新生儿分为第2天移植组(D2组)、第3天移植组(D3组)、第4天移植组(D4组)、第5天和第6天移植组(D5-6组),进一步分析胚胎体外培养时间对新生儿性别比例的影响。结果新鲜胚胎移植周期出生男性婴儿2 128名(54.41%),解冻胚胎移植周期出生男性婴儿780名(52.55%),两组比较差异无统计学意义(P>0.05)。IVF组新生儿3 355名,其中男性1 839名(54.81%);ICSI组新生儿556名,其中男性289名(51.98%),两组比较差异无统计学意义(P>0.05)。随着移植胚胎在体外培养时间的延长,新生儿中男性婴儿的比例由D2组的50.68%逐渐上升到D5-6组的56.78%,但组间比较差异无统计学意义(P>0.05)。进一步分析各组移植胚胎中优胚率,发现D4组和D5-6组优胚率最低,显著低于D2组和D3组,差异均有统计学意义(均P<0.05)。结论相比于IVF治疗,ICSI治疗和胚胎冻融处理可降低新生儿中男性婴儿的比例,但随着移植胚胎在体外培养时间的延长,活产男婴的比例逐渐升高。Objective To analyze the effect of assisted reproductive technology on neonatal sex ratio. Methods The data of patients who were treated with assisted reproductive therapy and had successful pregnancies and deliveries since 2012 in Reproductive Medicine Center of the Third Affiliated Hospital of Guangzhou Medical University was followed up,4 062 cycles were included. And firstly,the neonates were divided into fresh embryo transfer group and frozen thawed embryo transfer group,and the neonatal sex ratios of two groups were compared. Secondly,the neonates in fresh embryo transfer group were divided into in vitro fertilization( IVF) group and intracytoplasmic sperm injection( ICSI) group according to assisted reproduction technology,and the neonatal sex ratios of two groups were compared. Finally,according to the culture time of transplanted embryos in vitro,all the neonates were divided into D2 group,D3 group,D4 group,D5-6group,and the effect of embryo culture time in vitro on neonatal sex ratio was further analyzed. Results There were 2 128 male infants( 54. 41%) born in fresh embryo transfer group and 780 male infants( 52. 55%) born in frozen thawed embryo transfer group,and there was no statistically significant difference between the two groups( P〉0. 05). There were 3 355 infants in IVF group,and 1 839 male infants( 54. 81%) were included; there were 556 infants in ICSI group,and 289 male infants( 51. 98%) were included,there was no statistically significant difference between the two groups( P〉0. 05). With extension of embryo culture time in vitro,the proportion of male infants in neonates increased from 50. 68% in D2 group to 56. 78% in D5-6 group,and there was no statistically significant difference between the two groups( P〉0. 05) According to the further analysis of high-qualified embryo rate in each group,the high-qualified embryo rates in D4 group and D5-6 group were the lowest,which were statistically significantly lower than those in D2 group and D3 group( al
